1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On 02/21/2025 at 10:00 am, Licensing Program Analyst (LPA), Elka Chavez conducted an announced Case Management inspection for a change in capacity. Upon arrival, LPA met with Licensee Representative, Irma Luna. This facility is currently licensed to serve 36 preschool children ages 2 to 5 in classrooms: (1) Busy Bees, (2) Science, (3) Koalas and (4) Buddy Bears. The Licensee, Lutheran Church of the Good Shepherd, is requesting to reduce the existing preschool capacity to 20 preschoolers serving ages 2 to 5 years old in classroom (4) Buddy Bears. Hours of operation will be Monday to Friday from 7am to 6pm. The facility is located on the premises of the Good Shephard Lutheran School. There is an operational private elementary school and child care center with license number (198021670). There is a parking lot available to parents for drop off and pick up off Ruby Street and Avenue 62. The parking lot gates are open from 7:00am to 9:00am. After 9:00am the parking lot gates are closed, and parents are required to use the schools main entrance located off Garvanza Avenue. There is a doorbell with a camera that alerts staff to open the door for parents/authorized representatives. The preschool has its own designated entrance to classroom (4) which is separate from the Private Elementary school and Rayuela School (198021670). Present during today’s inspection were 7 children with 2 staff. Appropriate ratios and supervision were observed. All indoor and outdoor activity space utilized for the children was inspected today. LPA informed Irma Luna that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, Irma Luna stated that they will be stored in a top cabinet in the office area. A fully equipped first aid kit is located in the classroom. There is an operational carbon monoxide detector on site located in the classroom (4). All required licensing documents were observed posted next to the of the classroom (4). Children will be signed in and out at the lunch tables and parents escort the children to their designated classroom.

LPA continued to tour the facility and obtained the measurements for classroom (4) Buddy Bears which will only be used by the facility. The total indoor activity space for the preschool program measured 709.03 square feet, which is sufficient to accommodate the requested capacity of 20 preschool children. LPA observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots, bookshelves, and other activity supplies for the children. Drinking water is available in the classrooms via bottled water. LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on 02/19/2025.

LPA observed a total of 6 sinks, 3 toilets and 1 urinal available to the preschool children. These are sufficient to accommodate the requested capacity of preschool children. There is a separate staff restroom equipped with a toilet and a sink that is accessible to staff next to the kitchen area of the elementary school. The isolation area for children who are ill will be the main office located within the chapel. The restroom in the chapel is designated as the ill children restroom.

Facility will provide dry AM/PM snacks. Parents provide their child’s lunch. The classroom is equipped with a sink, food prep area and microwave. The classroom’s prep area was observed to be free of hazards. The food storage areas were observed free of rodents and/or vermin.

There is a fully fenced playground area that is currently designated for use under the preschool license of Good Shepherd CDC. The fencing surrounding the playground area is made of wrought iron fencing which was observed to be at least four feet high. LPA obtained measurements of the designated outdoor area which calculated to be 2760.35 square feet, which is sufficient for the requested capacity of 20 children. A playground waiver to share the playground at different times in the AM and PM with Rayuela School was submitted and approved by the Department. There are sufficient outdoor age-appropriate toys and play equipment available within playground area. There is adequate cushioning in fall zones of climber provider via rubber outdoor mats. Shade is provided via canopies and fixed shade structure. Drinking water will be available via water bottle. LPA observed all hazardous items on the playgrounds to be inaccessible to children.
