25 | Licensing Program Analyst (LPA) Dayna Chambers conducted an unannounced annual inspection to the above facility on 03/12/2025. LPA arrived at the facility at 1:25PM, identified self, and met with Lolita Willis, Licensee, who guided analyst on a tour of the facility. LPA provided Licensee with a copy of the LIC 126 Entrance Checklist to help facilitate the inspection. LPA observed that also present during this inspection, was Ashley Ruiz, Jessenia Ruiz-Chavez, and Melissa Perez Ruiz, Licensee’s Assistants. Per Licensee, they currently have 8 assistants (not all full time). LPA observed 8 children upon arrival. There are 22 children that are currently enrolled. Children attend different hours. A current LIC9040 children’s roster was available for review. Per Licensee, operation hours are 6:00am to 6:00pm Monday through Friday. Individuals who reside in the home were noted and discussed.
This is a one-story home which consists of: This is a one-story home which consists of 3 bedrooms, 2 bathrooms, kitchen, dining room, laundry room, living room, front yard, and backyard (fenced) and attached garage which is converted into an activity room. In the backyard, LPA observed an apartment unit that has the 1722 Gladys Ave, LB. Tenant has a background clearance. The children use the bathroom in the hallway near the bedrooms. Per Licensee, areas off limits to children and parents include: 3 bedrooms, 1 bathroom, and backyard apartment unit. The licensee provides food for children in care. Individuals who reside in the home were noted and discussed. Per Licensee, they currently have 8 assistants. Children use the following: The Livingroom, kitchen/dining room, converted garage room, The restroom that children use was observed to be safe and sanitary. Currently, children are using the back yard for outdoor play time. The outdoor play area was observed to be fenced. LPA observed that the outdoor yard has toys and other materials for children to play with. LPA did not observe any objects that can pose a danger to children on the outdoor yard. LPA did not observe any infant walkers, baby bouncers, inclined chairs, baby saucers, or any similar items at the facility. Safe toys play equipment and materials were observed. All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness.
