25 | Licensing Program Analyst (LPA) Judy Mora conducted an unannounced Case Management-Deficiencies inspection on this date. LPA met with Rocio Urias, Infant Teacher. Interim-Director, Veronica Rafael, was not present during today's inspection. LPA guided herself on a tour of the facility at approximately 8:40 AM due to no staff being available to guide LPA. This is a combination center with an infant license on site, license number 198018723.
The purpose of this visit was to ensure that the Licensee is in compliance with Title 22 regulations and the Health and Safety Code.
LPA observed 13 children present in the preschool classroom with two staff and five children with one staff in the Toddler Two classroom. Staff names were recorded. Criminal Record Clearances were reviewed.
LPA observed that two staff, Emely Galdamez and Nancy Clara, do not have a criminal record clearance on file; nor are they associated to the infant license. Ms. Galdamez has been working in the facility for approximately four months and Ms. Clara has been working in the facility for approximately three months. An immediate civil penalty is being assessed. This is an immediate risk to the health and safety of children in care.
Upon receipt of this report, the licensee shall post ANY licensing report documenting a type “A” citation. This must remain posted for 30 days during hours of operation. In addition to posting this report, the licensee will also provide copies to the parents of the children in care for up to one year.
A copy of the LIC 9224 - Acknowledgement of Receipt of Licensing Reports was provided.
Exit interview was conducted with Veronica Rafael. Appeal rights and procedures were explained.
