25 | Licensing Program Analyst (LPA) Judy Mora conducted a case management inspection due to an incident that occurred on or around 04/03/2023. LPA met with Assistant Site Supervisor, Veronica Rafael, who guided LPA on a tour of the facility. At the time of inspection, LPA observed 17 children with 02 staff in the preschool room and 12 children with 01 staff in the Toddler 2 Room. Staff to children ratios were observed to be in accordance with Title 22 Regulations. LPA conducted interviews and obtained documentation during this visit.
The incident that was reported to the Department on Monday, April 3, 2023 was not reported to the Department until 04/24/23 via email. Due to mis-communication the incident was not reported within the required 24hrs. The facility is being cited for failure to report this incident to the Department within the required 24 hours. The facility was cited for Reporting Requirements in August 2022. This was a repeat violation within 12 months; due to the repeat violation the facility will be assessed civil penalties.
Based on all information obtained on this date, and interviews conducted with teachers and a child, no follow-up is necessary regarding the incident. The incident appears to be an unusual incident. There were no disclosures made about the incident to corroborate with the allegation of personal rights.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
The deficiency listed on the following page was observed by the LPA and is being cited in accordance with California Code of Regulations Title 22. Please see attached LIC 809d.
