On 10/10/2024 at 12:15 pm Licensing Program Analyst (LPA) Carolyn Tuba conducted an unannounced case management licensee-initiated inspection. A Covid assessment was conducted. LPA arrived and met with staff member, Mahvel Gala-Zuvala. Shortly after Child Development Supervisor: Kathy Jarvey arrived at approximately 1:15 pm to further assist with the visit. LPA observed that there were no children present. According to staff member 15 children are currently enrolled. The preschool program’s hours of operation are Monday thru Friday, 8:00 am to 11:30 am.

This facility is located on the campus of Sunset Elementary School and is part of the Hacienda La Puente Unified School District. This is a preschool program which was licensed to operate 1 preschool classroom at a capacity of 18 children. Manager, Monica Jarvis notified the Department with an Unusual Incident Report (UIR) on 9/11/2024 advising that due to the air conditioning and heating system not working properly in the current licensed classroom #2, the children have been moved to a temporary classroom #511A beginning 9/10/2024. The children will be in this classroom until 5/21/2025. There is an empty classroom #507A next door which is not being used and is separated by the Teacher Workroom. According to the Child Development Supervisor this licensed facility will be closed at the end of the school year. LPA advised of the procedures for closing the preschool program. The classroom #511A has a Teacher Workroom with a staff restroom located inside. There is currently a storage cabinet for staff to keep personal belongings and school supplies. LPA observed children’s toys and furniture were appropriate.

Currently children have access to drinking water by the use of water bottles from home and refills are provided by bottled water. Meals are delivered by the School District Food Service and are provided Breakfast and Lunch. Children do not nap at the facility, as they are dismissed by 11:30 am. First Aid Kit is available.

LPA observed 2 child size toilets and 1 child size sink in the classroom, however due to the current capacity of children that the facility is licensed for, LPA advised that an additional sink would need to be added per the regulation, 1 toilet and 1 sink for every 15 children. According to Child Development Supervisor they have decided to cap at 15 students so the current capacity will need to be reduced.

There is a carbon monoxide and smoke detectors that are connected to the Fire Department and are tested through-out the year. The last annual maintenance on the Fire Extinguisher was on 7/1/2024.

There is a mini refrigerator and sink, located inside the Teacher’s Workroom which is locked when children are present. Cleaning supplies also kept on the top shelf, as well as any medications in the Teacher’s Workroom or refrigerator as needed and will be out of children’s reach.

Parent Board is located outside with all required licensing forms, including meal menus and parent sign-in/out sheets are kept on the rolling parent communication cart.

During this inspection measurements were obtained for indoor and outdoor space to determine capacity of the classroom and playground.

Classroom 511A (Preschool) measured for a capacity of 37 children, however due to the toilet and sink accommodations a capacity of 15 children will be allowed.
Preschool outdoor space measured for a capacity of 106 students.

The playground was inspected and is enclosed with a chain link fence and is separate from the Elementary School which is currently closed due to low enrollment. The signage on the playground equipment appears to be age appropriate, however the signage states ages 5 to 12 years of age. (LPA took photos of sign and play equipment. Staff member stated that when children are outdoors, they reframe from using the play equipment/structure and bring out age-appropriate outside equipment. There are trees for shade and outside has grassy areas. The structure and swing set have a cushion padding underneath.

LPA consulted and corrections need to be made for Classroom 511A’s usage:
Outdoor Water Fountain requires Lead Testing – Per Child Development Supervisor water fountain will be closed off until testing is conducted.
School District Fire Clearance will be provided by Manager, Monica Jarvis.
Play Equipment will be off-limits until further investigation is conducted by the facility to make sure the equipment is age appropriate as the signage states ages 5 to 12 years old. LPA will complete a final review before issuing a new license.
Child Development Supervisor will submit new facility sketch.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Child Development Supervisor, Kathy Jarvey.
