25 | On Thursday, September 1, 2022 at 11:40 a.m., Licensing Program Analysts (LPAs) Mayra Rivera and Patricia Medel conducted a Case Management inspection at the above facility to follow up on the self reported incident that occurred on 3/30/22, 5/20/22, 6/9/22 and 8/22/22. LPA’s met with Jazmin Munoz Peralta, Site Supervisor who guided the LPA’s on a tour of the facility The Monterey Park South West Child Care Regional Office received the incident report via email on 3/24/22, 5/20/22, 8/23/2022, and 6/14/22
LPA Rviera interviewed staff, reviewed documentation, and obtained copies of relevant documents. determining whether or not a violation occurred.
Base on the information provided and documents received in regards the incidents that occurred on 3/30/22 and 6/9/22, the facility followed procedures to ensure children were in a safe lighted environment. Therefore, LPA Rivera determined there to be no violation of Tittle 22.
Base on the information provided and documents received and statements in regards the incidents that occurred on 5/20/22 and 8/22/22, the children were being actively supervised and proper procedures were followed, therefore LPA Rivera determine there to be no violation of Tittle 22.
Upon receipt, Notice of Site Visit shall be posted for thirty (30) consecutive days where the parent/guardian of children enter and exit the facility Failure to maintain posting as required will result in a $100 civil penalty.
Exit interview conducted with Site Supervisor Jazmin Munoz Peralta and appeal rights were provided and explained.
