13 | Licensing Program Analyst (LPA), T Tran arrived at the above licensed facility to conduct an unannounced subsequent complaint inspection for the purpose of concluding the investigation of the above allegations. Upon arrival, LPA met with Site Supervisor, Isabel Vitela and toured the facility. LPA observed proper care and supervision.
Based upon the evidence obtained during the course of the investigation through record reviews, observation, and interviews with staff, children, and others. The evidence does not support, nor disprove the above allegation, staff do not follow C1 (see LIC811) medical care plan and staff are not accepting C1 back into the facility after the medical updated until a couple days later. Records reviewed indication at the beginning of October 2025, the facility had a meeting with C1's parent regarding switching medication to update C1 medical care plan. This process took two days for the school nurse to review and clear the child before returning to school. Therefore, the allegations have been determined unsubstantiated. Unsubstantiated – A finding that the complaint is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. This report was reviewed with the facility representative, Isabel Vitela.
