Licensing Program Analysts LPAs Staicy Perry and Carolyn Tuba conducted an unannounced 3-Yr random inspection to the above facility. LPAs met with licensee Alba Luz Guerreiro. The licensee is primarily Spanish speaking. LPA C.Tuba assisted in interpreting. During the visit licensees husband George Guerreiro, granddaughter Diana Alemannas as well as son George Guerreiro. The licensee states that she has currently has 11 children enrolled. A current children’s roster is available and is current. Licensee’s hours of operation are 6:00am-7:00pm, Monday-Sunday.

The licensee provides care in the home, which is a one-story home with 2 bedrooms, 2 bathrooms, kitchen, dining room and living room. The home also has a detached recreation that has been permitted by the city. The recreation area consists of 3 rooms and a bathroom. Per Licensee, detached recreation structure in the back yard is no longer used for daycare. Detached recreation is being occupied by son. Licensee did update LIC 999A has been updated and reporting responsibilities were reviewed during today’s inspection. All areas identified on the facility sketch were inspected. The licensee provides food for children in care. Licensee provides lunch, PM Snack and Dinner.

The licensee states that 4 adults and 2 minors currently live in the home. Persons living in the home are identified on the attached LIC811. All individuals present in the home have obtained a criminal record clearance or exemption prior to working, residing, or volunteering in a licensed home. Per licensee, there are no weapons, firearms, or bodies of water on the premises.

LPAs were led on a tour of the facility by the licensee. Parents/Guardians enter through the front door of the backdoor through the gate. LPAs reviewed the living room which is the primary area for childcare. There were 2 children present during the inspection with one being licensee’s granddaughter. LPAs observed age-appropriate toys and equipment present. Emergency Disaster Plan, Parent’s Rights Poster and the Facility License were not observed to be posted. LPAs provided client with all missing documents and upon departure licensee had sign posted visible for parents to see. The tour continued to the dining area where toys were observed along with a diaper changing area with adequate supplies for diaper changing. Tour continued to the kitchen. Sharp knives are made inaccessible by safety latch on a drawer. A fire extinguisher was observed. The valve on the required 2A 10BC fire extinguisher indicates fully charged and was serviced on 8/15/2023, as indicated on service tag. Per State Fire Marshall standards, fire extinguishers shall be serviced annually. LPAs discussed the importance of ensuring the fire extinguisher is serviced or a new canister is purchased annually. The bathroom #1 which is in bedroom #1 was reviewed next. Detergents, cleaning compounds medications, and other items which could pose a danger are accessible to children. Medications were found on accessible. LPAs observed medication in bedroom #1 drawer that is accessible to child as well as bedroom #2 on vanity, night stands, and drawers. LPAs informed licensee that medications need to be made inaccessible to children. Medications should be placed in a locked cabinet made inaccessible to children. LPAs also observed laundry detergent to be accessible to children in the kitchen floor near the fridge. LPAs gave instructions to licensee an LPAs observed licensee place detergent in an inaccessible cabinet area. The tour continued into the back yard. The outdoor play area was observed to be fenced along with a kiddie gate. Front yard is not used for the daycare.

The licensee states that there are no poisons in the home and understands that storage areas for poisons must be locked with a key or combination lock. There are first aid supplies available. Smoke and carbon monoxide detectors were tested and are operable. Licensee uses landline. Per licensee, bean bag in living room will be used as an area for an ill child until an authorized representative comes to pick up child. LPAs observed napping equipment in the form of cots for the children in care. LPAs recommended the use of a crib or play yard for the size of the child in care. An advisory Note is being provided. The licensee is observed to be operating within the license capacity limitations

The licensee and other personnel have completed training on preventive health practices including Pediatric First Aid and CPR. The licensee's Pediatric First Aid and CPR expires on 4/2025. Licensee was not able to provide proof of Mandated Reporter Training. The assistance’s Pediatric First Aid and CPR expires on 4/2025, as well as Mandated Reporter Training that expires on 7/6/2025 as well as assistants’ proof of immunization against influenza, tuberculosis, pertussis, and measles were reviewed and on file. Children’s records were reviewed, including but not limited to, a copy of the emergency information card that contains all the information specified by regulation.

-There is 1 small dog in back recreation structure and 1 large dog located in the dog run outside of the premises located on the side of the house.


-Infant Walkers, Johnny Jumpers, Saucer Chairs, and/or any other item that falls into these categories are not permitted in a family childcare facility. Smoking is prohibited in a license family childcare home.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee [or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, the Licensee, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

Based on the LPA’s observations and records review, the following deficiencies listed on the attached LIC 809 (deficiency page) are being cited in accordance with California Code of Regulations Title 22.

Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

A notice of site visit was given and must remain posted for 30 days.

Exit interview conducted and report was reviewed with the licensee Alba. Plan of corrections was developed and reviewed. Copy of report provided.
