25 | On March 4, 2026, at approximately 10:00 a.m., Licensing Program Analyst (LPA) Janet Gil conducted a case management inspection and met with Director Sandra Magdaleno. The purpose of the inspection was explained to the Director.
The facility operates under a dual license. During today’s visit, for the preschool license, there were 10 staff members present (including the Director) and 54 enrolled children. For the infant license, there were 7 staff members present and 20 enrolled infants. The facility was operating within its licensed capacity and in accordance with the required staff-to-child ratios. All adults working in the facility have a criminal record clearance on file.
This case management inspection was conducted in response to the Unusual Incident Report (UIR) submitted on February 23, 2026. LPA discussed the nature of the incident during the inspection. The incident occurred on February 23, 2026, when a child (C1) was left unattended in a preschool classroom. Per the Director, C1 was located within approximately 30 seconds and was found in a neutral state.
Per the Director and the involved staff member, the facility required the involved staff to complete coaching regarding child supervision, which took place on March 3, 2026. The involved staff members will also complete online retraining on supervision and proper transitions.
LPA conducted an inspection of the facility, both indoors and outdoors, for health and safety hazards.
Based on information obtained during today’s inspection, a Type B deficiency is being cited under California Code of Regulations (CCR), Title 22, Division 12, Chapter 1, for lack of supervision. Refer to LIC 809-D for the cited deficiency.
