32 | Allegation #1: Day-care child sustained an unexplained injury while in care.
According to the allegation, Child #1 had unexplained bruises on the side of the head and a swollen and bruised eye. During interviews conducted, staff stated parents are notified of any injuries to their child and an ouch report is given to the parent. Staff stated parents are notified for injuries such as bruises, bleeding, scratches, cuts, abrasions, bites, etc. Director stated that even if there is no visible injury, "we do an ouch report and we still take a picture and send it to the parent. If they hit their head we let the parent know even if there is no visible injury". Staff #3 stated, "Any injuries...anytime a child is going home differently than when they came, we let the parents know". Staff #2 stated the facility notifies parents of any injuries, "Any injury that has to do with bleeding, bruises, bumps on the head, eye issues. Anything we see we let the parents know. We notify parents on the ProCare app and we put pictures on there and we also give them an ouch report". Staff interviewed stated there has never been a time child received an injury while at the facility that should have been reported to parents and was not reported. LPA reviewed the facility files for children in care and did not observe any ouch reports in the file for Child #1. Parents interviewed stated they are happy with the care and supervision of their children and parents stated they are notified by the facility via an Ouch Report and phone call if their child receives an injury. Children interviewed stated they liked their teachers. Children interviewed did not disclose any mistreatment by any staff or other children at the facility.
Allegation #2: Staff did not feed a child in care.
Complaint alleges staff were not feeding Child #1 and Child #1 would not eat the food brought from home. Staff interviewed stated the facility provides a morning snack, lunch, and an afternoon snack, and parents can send additional food to the facility for their child to eat. Staff stated parents are notified how much their children eat while at the facility through a daily note on the ProCare App. Staff #1 stated "staff monitor to make sure children are eating something at least. Some kids are picky and we try to offer them fruit or an alternative snack if they don't eat their food". Staff #2 stated if a child does not want to eat the food that is provided by a parent or the facility, "we try to give them alternatives of things we know they like to eat. We try to offer them whatever we can". Staff #2 stated, "I take picture's of them eating to
