On 03/26/26, at 8:30 AM, Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced complaint inspection in regards to the above allegation. LPA met with Shonna Clark, director who guided analyst on a tour of the facility. During this inspection there were total of 8 infants present at the facility with 3 staff.

During this investigation LPA obtained a copy of current facility roster.

During 03/16/26 inspection director, Shonna Clark checked the facility website and confirmed the facility number is not posted on the website.

LPA Eivazian printed the facility website home page on 03/12/26, and facility number was not posted on the facility website.

Based on LPA observations and staff#1 interview, the preponderance of evidence standard has been meet, therefore the above allegation is found to be Substantiated at this time. Title 22 Regulation code 101162 (1)(a) Advertisements and License Number--Licensees shall reveal each child care center license number in all advertisements in accordance with Health and Safety Code Section 1596.861 is being cited on the attached LIC 9099D.

The following deficiencies listed on the attached LIC 809 (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiency that is being cited need to be cleared to protect the children’s health & safety.

The Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days.
