On January 9, 2025, at 9:40AM, Licensing Program Analyst (LPA) Keneisha Dunlap conducted an unannounced annual required visit at the above facility. LPA Dunlap announced the purpose of today’s inspection, and was granted entry into the facility by Director-Patricia Diaz. Director conducted a guided tour of the facility inside and outside with LPA Dunlap. There are 3 staff in classroom. 23 children enrolled and 21 children present. LPA observed proper care and supervision. All center staff that were present during today’s inspection had fingerprint cleared and associated to the designated license number.

This is a Preschool program from 7:00am - 6:00pm Monday - Friday. LPA observed all posting requirements for operation on the licensing board LIC 203A-License, LIC 610 A-Emergency Disaster Plan, LIC 9148-Earthquake Preparedness Check List, LIC 613A- Personal Rights, Menus, Activity Schedule, and completion testing and remediated lead exceedances.
Documentation of current Fire & Earthquake drill was observed with last drill on 12/4/24.
Fire extinguisher observed with a tag date of 2/27/24.
At 10:15a.m., LPA Dunlap observed the licensing board missing the following:
PUB 393-Notification of parent’s rights
PUB 269- Child Car Seat Law
This poses a potential health and safety risk to children in care. Technical Assistant will be issued.
LPA Dunlap provided the Director with a copy of the PUB393 and PUB269 during inspection. LPA Dunlap observed the Director posting both forms on the licensing board.

LPA observed a operable fire system, and carbon monoxide detector.

LPA inspected the classroom equipped with age appropriateness furniture and learning materials. Telephone service, heating, lighting, and ventilation were evaluated. Children do nap and the cots are stored in the

closet in the classroom. Storage for children's belongings, manipulative area, dramatic play area, library, arts & crafts area, and working bathrooms facilities. The classroom has a fish tank with a small fish on counter. The isolation area is located in the Directors office. Facility has igloo with water and disposable cups accessible indoor for children to use. All electrical outlets are covered. First Aid supplies were inventoried. A review of medication policy, including administering, labeling, storage, and records was made. All cleaning supplies and/or hazardous materials stored in cabinets and drawers with safety latches.

LPA Dunlap observed sign in and out sheets for all required signatures for children in attendance.

Outdoor play area observed and inspected for safety, cushioning material, good repair and age appropriateness. Required shade, drinking water and fencing were inspected that met the regulation. Play area was inspected for hazards and inaccessibility to bodies of water.

All food items are delivered daily from the main kitchen of the company. The facility does provide breakfast, lunch, and snack. LPA Dunlap observed the kitchen area where food is distributed to be clean and organized. Knives are stored in drawer with safety latch.

Facility is reminded that smoking is prohibited on the premises.

Criminal Record Clearance - Child Care Centers- Director- Patricia Diaz was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Lead Testing

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

Incidental Medical Services (IMS)

This facility provides Incidental Medical Services – IMS.

LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. For IMS information see PIN 22-02-CCP. Director- Patricia Diaz was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

A notice of site visit was given and must remain posted for 30 days.
