13 | Licensing Program Analyst (LPA) Judy Mora conducted an unannounced initial 10 day inspection to investigate the above complaint allegation. LPA toured facility with Director, Michelle Johnson at approximately 11:35 AM. LPA observed 20 children with 02 staff on the playground.
During the course of this inspection LPA visually observed the children and staff on the playground to determine if needs of children and ratios are being met. LPA inspected the facililty both indoors and outdoors. LPA also obtained a copy of the facility roster. Based on the available information, the preponderance of evidence standard has been met, therefore the above allegation is found to be Substantiated. California Code of Regulations,(Title 22, Division 12 & Chapter Number 1), are being cited on the attached LIC. 9099D. LPA observed several items including equpment and materials stacked in the hallway outside of the classroom. LPA also observed stacked items such as trash cans and crates next to the sink in the toddler room. This poses a potential Health & Safety risk to clients in care. Photos were taken.
