13 | Licensing Program Analyst (LPA) Thelma Razo conducted a subsequent complaint visit to deliver findings on the above allegation. LPA met staff with and stated the purpose of the visit. Upon arrival today 9/24/2021 at 10:04AM, LPA observed 3 infants with teacher Fukiko Sanders.
On 8/20/2021, LPA held an Initial 10-Day Visit and tour the facility which consists of two infant rooms and outdoor play area. LPA interviewed 3 staff and obtained children roster, staff work schedule, and Declaration from 3 infant staff dated 8/19/2021. LPA interviewed 2 parents and one staff over the phone.
Upon LPA's arrival at the infant day care on 8/20/2021 at 9:04AM, LPA observed one staff with four children in the outdoor play area. Supervisor Alicia Flores provided LPA with a copy of Declaration from 3 infant staff indicating that they went through a training regarding supervision after the facility was cited previously. Various interviews have revealed that staff have undergone training and watched video and understood that infants should not be left unsupervised. There were no disclosures or witnesses to support the allegation.
