On Friday May 3, 2024, at 10:37 am, Licensing Program Analysts (LPAs) Staicy Perry and Portia Bowden conducted an unannounced annual inspection and met with Ana Marquardt, Associate Director of Education, Carlos Tobar, Quality Assurance Director and Golnaz Kooklanfar, Coach Coordinator who guided LPAs on a tour of the facility.

This is infant program licensed for 63 children which operates Monday – Friday from 7:30 AM – 3:00 PM. Per Ana Marquardt, Associate Director of Education, there are 42 children enrolled. The facility has a toddler option of 27 children ages 18-36 months. There is an preschool license on site, #198011690. Infant and toddler component are conducted in physically separate areas both indoors and outdoors. Sign in and out takes place at the main entrance to each classroom. All areas identified on the facility sketch were toured and inspected both indoors and outdoors. Teacher and child ratio were observed, and present staff are fingerprinted cleared. Census was taken and LPA’s observed 26 children (20 toddlers and 6 infants) and 15 staff.



At approximately 10:50 am. LPAs entered POD 1 (0-12months). LPAs Perry and Portia observed 4 children and 2 staff present supervising the children. LPAs observed age-appropriate furniture and equipment to be in good condition, free of sharp, not loose, or pointed parts and age-appropriate toys accessible to children. LPAs observed a cubby for children to store their personal belongings. LPA observed cleaning solutions stored inside the top cabinet with a key lock making it inaccessible for children to reach and open. LPAs Perry and Bowden observed a step trash bin with a lid. LPA’s observed cameras in the room.

At approximately 11:00 am. LPAs entered POD 2 (12-18 months). LPAs Perry and Portia observed 1 child and 1 staff present supervising the children. LPAs observed age-appropriate furniture and equipment to be in good condition, free of sharp, not loose, or pointed parts and age-appropriate toys accessible to children. LPAs observed a cubby for children to store their personal belongings.

At approximately 11:54 am. LPAs entered POD 8 (24-36month). LPAs Perry and Portia observed 1 child napping and 1 staff present supervising the children. LPAs observed age-appropriate furniture and equipment to be in good condition, free of sharp, not loose, or pointed parts and age-appropriate toys accessible to children. LPAs observed a cubby for children to store their personal belongings. LPA observed cleaning solutions stored inside the top cabinet with a key lock making it inaccessible for children to reach and open. LPAs Perry and Bowden observed a step trash bin with a lid. LPA’s observed cameras in the room. LPA’s observed laundry room made inaccessible to the children.

Heating, lighting, and ventilation were evaluated by LPAs Perry and Bowden. LPAs observed central AC/heater and vents located on the ceiling. LPAs observed the thermostats located in all each class, POD 1-POD 8 temperatures to be between 72-73 degrees. LPAs observed the fire extinguishers located in PODS (classrooms) and hallways and the valve on the required 2A10BC fire extinguishers indicating fully charged and serviced on 8/22/23. Smoke detectors were not tested during this visit due to the detectors being wired to the emergency pull down fire alarm system. Carbon monoxide detectors were observed to be in all PODs (classroom), LPAs tested and are operable. The last emergency drill was conducted on 4/24/24. LPAs observed napping equipment cots to be in good condition and stored in the classrooms. First aid kit supplies are kept inside the PODs (classrooms) and LPAs observed the kits to be fully equipped. During this visit, per Ana Marquardt, Associate Director of Education, stated currently has 1 child with medication or severe food allergies. LPAs asked if there were any poisons, firearms, weapons, or bodies of water. Ana Marquardt, Associate Director of Education stated no firearms, weapons, no poisons nor bodies of water. LPAs did not observe firearms or weapons, poisons, nor bodies of water. Lead teacher Edith stated the ill isolation area takes place in the center managers office and children will utilize staff restroom.

POD 4 (classroom) and POD 5 (classroom) share restroom and the restroom is in the hallway near POD 4. POD 7 (classroom) and POD 8 (classroom) share restrooms and the restroom is located between both PODs (classroom). LPAs observed the restrooms to be in good condition with working toilets, sinks, hand soap dispenser and paper towels. LPAs observed toilets and hand washing sinks.

At approximately 12:00 p.m. LPAs entered the Infant outdoor play area. LPAs observed the equipment to be in a safe condition, free of sharp, no lose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. LPAs observed play equipment to be age appropriate and areas around or under high climbing equipment, are cushioned that may absorb a fall. For adequate shade, LPAs observed outdoor patio coverage. LPAs observed the playground yard to be fenced and the side gates closed and locked with a keypad lock and alarm on the door. LPAs observed trash bins with tight lids. For outdoor water drinking, staff bring out water jugs and disposable cups.



At approximately 12:04 p.m. LPAs entered the toddler outdoor play area. LPAs observed the equipment to be in a safe condition, free of sharp, no lose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. LPAs observed play equipment to be age appropriate and areas around or under high climbing equipment, are cushioned that may absorb a fall. For adequate shade, LPAs observed outdoor patio coverage. This program provides breakfast, lunch, and pm snack. Meals and snacks are provided by their central kitchen. LPAs informed Ana Marquardt, Associate Director of Education that any food brought from the children's homes, the container shall be labeled with the child's name and properly stored or refrigerated. For water drinking, LPAs observed water jugs and disposable cups in each POD (classroom). LPAs observed the food preparation area to be clean and free of litter, rubbish, rodents, and/or any other vermin and storage containers for solid waste, including movable bins with tight-fitting covers that are kept on, and in good repair. The facility was observed to be free of flies, other insects, and rodents.

observed 42 signatures. Lead testing was conducted on 11/17/22 and no lead exceedance. Lead results were posted and observed by LPA’s.

LPAs reviewed the children’s roster, children’s files and staff files and observed the files to be complete.



LPAs observed Center Mangers Berenice Perez-Pacheco Pediatric First Aid/CPR certification dated 3/2025. Preventative Health and Safety certification dated 10/4/23 and has proof of immunization against Pertussis, MMR, and Influenza declination. Director has completed the mandated reporter (AB 1207) training dated 8/7/25. Director was advised that the mandated reporter training must be completed every 2 years, and is available at www.mandatedreporterca.com


Items discussed during this visit:

1. The following items are prohibited by Licensing: Refused Entry to a Facility or Any Part of a Facility is a violation of Section 1596.852, 1596.853 or 1597.09. Regulations 101238 (g) (2), The Presence of an Excluded Individual, Fire Clearance Violations, Accessible Bodies of Water, Accessible Firearms, Ammunition or Both

2. Breakfast/Lunch/Snack Menus: Menus are required to be posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days must also be available upon request.

3. Current Children’s Roster: Each childcare facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. This roster shall be available to the licensing agency upon request.



4.LIC 311A - Records to be Maintained at the Facility: LPAs advised that forms, regulations, and quarterly updates can be accessed on the Child Care Licensing website at: www.ccld.ca.gov.

5.Pediatric First Aid and CPR: At least one person trained in Pediatric First Aid and CPR and EMSA approved must be present.

6.Designated Staff: The name of the childcare center director or fully qualified teacher(s) designated to act in the director's absence must be on file.

7.Qualifications: Educational background, training, and/or experience for each staff present must be available for review

8. Immunization Requirements: Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles or influenza declination

9.Children’s Records: must be available for review; including but not limited to, the following: Name, address, and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary.

Criminal Record Clearance - Ana Marquardt, Associate Director of Education was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.



Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1CCP).

LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

LPAs referred licensee Ana to the Department website for lead: Lead Toxicity Prevention and Water Testing Information.

Safe Sleep – LPAs discussed the safe sleep regulations with Ana and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPAs also informed licensee Ana of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Infant Care: LPAs reviewed Safe sleep regulations / documents, 15-minute sleep check documentation for infants 0-24 months were reviewed and on file for the department. LPAs observed and inspected sleeping equipment for infants. LPAs was informed by Ana that cribs are used for infants in care. LPAs observed that cribs do not hinder the entrance or exit to and from the space they are sleeping in. LPAs observed that crib is free from loose articles and objects. There are no objects hanging above or attached to the side of the crib. Mattresses were observed to be firm and covered with a fitted sheet that is appropriate to the mattress size. Each infant has their individual bedding and is washed weekly as needed. Soiled bedding is replaced when wet or soiled and is placed in an area inaccessible to infants. Ana was advised that infants shall not be swaddled while in care and all infants up to 12 months should be placed on their back for sleeping.

This facility provides Incidental Medical Services – IMS. LPAs reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records.

Currently, the facility is in compliance with California Code of Regulations Title 22. No deficiencies cited.
