25 | On Monday, December 15, 2025, Licensing Program Analyst (LPA) Staicy Perry conducted an unannounced case management inspection to follow up with an incident that was reported to the department. LPA met with Ana Marrquardt, Sr. Program Manager of School Readiness and Child Outcomes who guided LPA Perry on a tour of the facility. Census was taken. On Wednesday, 12/10/2025, it was self reported to the department that on Tuesday, 12.9.2025 it was brought to their attention that Staff #3 allegedly hit Child #1 on the cheeks during nap time.
During the visit, LPA Perry reviewed children’s roster and confirmed that C#1 was attending the facility during the day of the incident. LPA also conducted staff, child and parent interviews during the investigation. Additionally, LPA Perry obtained pertinent documents during the inspection.
During the interviews, Parent #1 stated that on an undisclosed date Child #1 disclosed that, staff #3 got both hands and touched C#1’s cheeks and indicated the incident occurred during nap time because she didn’t want to nap. Parent reported the concern to the Site Supervisor on 12/9/2025. During the child interviews, no disclosers pertained to the allegation. Interviews with staff revealed that Staff #1, Staff #2 and Staff #3 were assigned to the classroom during nap time. Staff #3 denied hitting Child #1 and denied using physical discipline. Staff #1 and Staff #2 stated they did not and have not ever observed any inappropriate physical contact between Staff #3 and Child #1. All staff interviewed stated there are no previous similar concerns involving Staff #3. Per interview and declaration from Ana Marquardt, facility did do their investigation and review of video footage. It was also noted that S#3 has been employed for 11 years and has no complaints and or allegations in regard to the above allegation. Facility dose have cameras and they have been reviewed and determined nothing happened. S#3 was put on administrative leave until the investigation was concluded and has since returned. LPA did observe S#3’s interactions with children and LPA did not observe any violation of a child’s personal rights.
Based on the information obtained, interviews conducted, and evidence reviewed there is not sufficient evidence to support the allegation. The facility is in compliance with Title 22 regulations. Facility did report the incident in a timely manner once they became aware of the incident. No deficiencies being cited on this date.
An exit interview was conducted with Ana Marquardt and Justine Francisco, Director of School Readiness and Child Outcomes a copy of this report was provided along with Appeal Rights. Notice of Site Visit was provided and must be posted for 30 days.
