13 | Licensing Program Analyst (LPA) Jeanette Estrada conducted an unannounced complaint inspection to the above facility. LPA met with Licensee, Silvia Preciado Camarena for the purpose of delivering the findings for the allegation above. At the time of arrival there were 5 children present with the Licensee and one assistant.
The allegation states that the Licensee hit a day care child. During the investigation, LPA conducted interviews and reviewed pertinent documents. Interviews revealed Child 1 attended the facility. Although Child 1 attended the facility, interviews conducted by LPA did not corroborate the allegation and no witnesses were identified. Licensee denied the allegation and stated she was not given a reason why Child 1 stopped attending. Therefore, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur; the allegation is unsubstantiated.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit made by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Licensee, Silvia Preciado Camarena. Appeal Rights were provided. |
