Licensing Program Analyst (LPA) Ashley Calderon conducted an unannounced annual inspection to the above facility. LPA arrived at the facility and met with Lashaun Merriman, Licensee, who was arriving at the facility from providing child care transportation. LPA gained entry to the facility.

Per Licensee, operation hours are Monday - Friday (Saturdays if needed) from 6am - 6pm. There were 6 children observed, including one infant.

This is a two-story home which consists of 3 bedrooms, 3 bathrooms, Living Room, Kitchen, Dining Room, Activity Room and yard. First Floor - 1 bathroom, living room, kitchen, activity room. Front yard and backyard and attached garage. Second Floor- 3 bedrooms and 2 bathrooms off limit to children. Children use: bathroom in the hallway next to kitchen, activity room and yard. LPA observed a fireplace in the off-limit living room and it is screened off.

Per Licensee, areas off limits to children and parents include: Second Floor rooms and kitchen. LPA observed the stairs to be barricaded for children under the age of 5. Indoor and out door area was observed for age appropriate toys. No bodies of water observed.

All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. There is telephone service via cellphone that is used, and the cellphone stays at the facility during operation hours. There is ventilation and heating (central heat and air). Safe toys play equipment and materials were observed. Detergents, cleaning compounds, medications, and other items which could pose a danger to children were observed to be inaccessible to children. The licensee states that there are no poisons in the home and understands that storage areas for poisons must be locked with a key or combination lock. The licensee provides food for children in care. LPA Calderon gave consolation and provided assistance with Safe Sleep Forms, Application update info and form, and posting forms. LPA will return at another time to continue annual inspection. Exit interview conducted with Licensee Merriman, Lashaun.
