13 | Licensing Program Analyst (LPA), T Tran arrived at the above licensed facility to conduct an unannounced subsequent complaint inspection for the purpose of concluding the investigation of the above allegation. Upon arrival, LPA met with interim Director, Leticia Olague. who provided LPA the tour of the facility. LPA observed proper care and supervision, and ratio.
Interview was conducted with staff. Based upon the evidence obtained during the course of the investigation through interviews, record reviews, and observation, the evidence does not support, nor disprove the above allegation of children were left unsupervised outside at the play area occurred. Per staff, the door to the playground is always closed when the children are inside. LPA observed the front and the back entry to the facility has the door alarm system. Therefore, the allegation has been determined unsubstantiated. Unsubstantiated – A finding that the complaint is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred.
No deficiency was cited at this time. A notice of site visit was given and must remain posted for 30 days.
