Licensing Program Analyst (LPA) Crystal Green conducted an unannounced required 1- year inspection. LPA met with designated facility Director, Cynthia Reimers. Due to COVID- 19 precautionary measures were taken, individuals present during inspection wore appropriate personal protective equipment. Facility was observed to be adhering to local public health guidelines. Licensee also has a preschool license 198012250 which is located on the same premises.

At approximately 12:05 PM LPA Green began inspection of the infant program. All areas identified on the facility sketch were inspected. LPA observed 6 children with 2 staff. Teacher-child ratios were observed to be in accordance with Title 22 regulations. The Licensee is within the conditions, limitations, and capacity specified on the license. Staff names were recorded. All children were observed to be under visual supervision of a teacher at all times.

Due to COVID- 19 precautionary measures, Director states that parents drop off and pick up will be conducted outside of the facility. Children are signed in upon entry to their designated classroom. Per Director, staff conduct pre screening prior to accepting children. LPA observed the facility to be clean, safe, sanitary and in good repair. Furniture and equipment were inspected for good repair, free of sharp, loose, or pointed parts. Children have their own cubby to store their belongings. Napping equipment was observed in form of cribs. Children's linens for the infant program are to be washed or replaced daily. There are age appropriate toys and supplies for children in the classroom, in good repair. There is a changing table within arm's reach of a sink in the classroom. LPA observed it to be clean. The facility was observed to have diapers and wipes available in each classroom near the changing table. Drinking water is available in form of personal water bottles that are also taken outside when needed.

There is a separate ill isolation area and restroom at the entrance of the facility. the office is used as an isolation area. Parents are contacted immediately when children are determined to be ill and staff are ensuring that children with obvious symptoms of illness are not being accepted.

The facility provides AM snack and PM snack. As a COVID-19 precautionary measure, Parents provide their child lunches. Infant formula is provided by parents. All bottles/cups observed to be labeled as well. Needs and Service plans were reviewed and observed to be updated. No children needing IMS services at this time. IMS plan provided and on file. All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair. Disinfectants, cleaning solutions, poisons and other items that are dangerous to children are stored in an area inaccessible to children. Storage areas for poisons are locked. Facility has one or more functioning carbon monoxide detectors that meet statutory requirements. The licensee takes measures to keep the facility free of flies, other insects and rodents.

Outdoor play equipment was observed to be in good condition, free of sharp, loose or pointed parts. Outdoor activity space surface is maintained in a safe condition as is free of hazards. Areas around and/or under climbing equipment, swings and slides have cushioning material to absorb a fall. LPA observed shade provided via canopies.

Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. Staff Records were reviewed to ensure that a health screening report is on file. Children’s Records were reviewed to ensure that Identification and Emergency form, a medical assessment, and Needs and Service Plan are on file. Criminal Records Clearance for adults and verification of CPR/First Aid and health preventative practices documentation was reviewed.


This facility provides Incidental Medical Services – IMS. Per Director, No children are currently requiring IMS services. At this time, the licensee is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued today.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
