25 | On Monday February 24, 2025 an unannounced Case Management - Plan of Corrections (POC) inspection was conducted by Licensing Program Analyst (LPA) Staicy Perry to ensure (10)Type B deficiencies issued on 1/2/2025 are cleared. LPA met with Dimanthi Deraniyagala, Director. Census was taken and LPA Perry observed 15 children in care with 5 staff.
Today, LPA observed:
- LPA observed the 3 out of 5 staff's Mandated Report Training to be on file and current.
- LPA observed and obtained staff meeting agenda, signatures and statements regarding Infant Feeding.
- LPA observed Director, Dimanthi Deraniyagala CPR certification which is from 1/2025-1/2027.
- LPA observed Infant sleeping logs and staff physically checking on sleeping infant(s) every 15 minutes and documenting for Infants up to 12 months of age who are sleeping in a position other than on their back.
-LPA observed outdoor play equipment to be clean and free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard.
- LPA observed diaper changing table to be within arms reach of a sink.
- LPA observed trash bins to have tight fitting lids.
- LPA observed 3 out of 5 staff to have immunization's on file.
- LPA observed 3 out of 5 staff to have LIC 503.
- LPA observed 3 out of 5 staff to have educational requirements on file.
LPA provided the facility with a "Letter of Deficiency Citations Cleared." Letter must be filed in facility for three years and upon request made accessible to the public for review. Per Chapter 3, Division 12, Title 22 Regulations no deficiencies are observed today. LIC9213 NOTICE OF SITE VISIT FORM IS REQUIRED TO BE POSTED FOR 30 DAYS.
