On 07/19/2023 at 10:40 AM an Unannounced Required – 1 Year Inspection was conducted by Licensing Program Analyst (LPA) Katrina Chicote. Licensing staff announced purpose of visit and met with Amber Johnson, Director, who granted entry to facility and provided tour of facility indoors. Facility is currently licensed for a capacity of 24, per Director they have 19 enrolled. The program currently operates Monday thru Friday 6:00 AM to 6:00 PM. LPA observed required licensing postings on Parent Board upon entry.

At the initial start of the site visit, there is a total census of 15 preschoolers and a total census of three teachers present. The facility is made up of four rooms, Movie Room (located to the right upon entry), Activity/Play Room (located upon entry), 2-3s Room (located down hallway to the rear of facility), Art Area/Breakfast Room (located next to 2-3s Room).

Beginning at 10:42 AM, furniture and equipment were inspected for age appropriateness and good repair. LPA observed carpets, and child size tables and chairs available for children well maintained. LPA observed learning stations for dramatic play, math, manipulatives and science with toys well maintained free of loose and sharp parts and a library area available for quiet play. Telephone service, heating, lighting and ventilation were evaluated. There were cubbies observed as storage for children's belongings. Mats/cots were inspected. Per Director, sheets are provided by facility and washed weekly and additional blankets are brought from home. Availability of drinking water was reviewed. LPA observed two water fountains available for children in Art/Breakfast Room. At 10:44 AM, LPA observed Bathroom (located in hallway between Movie Room and 2-3s Room) to have age appropriate sinks and toilets that were operable (3 sinks and 3 toilets), general area safety and sanitation, and adequate hand washing and toileting supplies at time of inspection.

Snack/lunch menus were reviewed and observed posted at time of inspection. Per Director, facility provides AM Snack, Lunch, and Dinner. Meals are delivered to facility in partnership with neighboring family child care home. LPA observed children eating on tables in Activity Room and observed trash cans used to discard food have tight fitting lids at time of inspection.

LPA observed a first aid kit is kept in the office. At 10:46 AM, carbon monoxide detectors were tested and observed operable at time of inspection. LPA observed smoke detectors observed present in the facility and fire extinguishers have been serviced on 07/19/2023. LPA observed disaster drill log available, last drill conducted on 05/10/2023. Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. LPA observed these items stored in cabinet with operable child safety locks in off limits Kitchen area, which is located through office. At 10:48 AM, LPA observed medicines that is stored in office for C1 and C2 to have unaltered label available. Director states that sick/isolation area is in office.

Beginning at 10:50 AM, outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. Required shade and fencing were inspected. LPA observed bicycles, a multi-level play structure with cushioning material below to absorb fall that are all well maintained. LPA observed plastic tables and benches, sand table, and other age appropriate toys available for children under shaded area and additional play structure with slide and circular swing on top of turf grass that is all well maintained. LPA observed a large avocado tree in rear area of playground that is well maintained. LPA advised that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to two drinking fountains available in the outdoor play area. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. LPA observed 12 of 15 children signed in at time of inspection and discussed observation with Director. Personal Rights of children were observed by LPA.

Beginning at 11:30AM, staff and children’s records were reviewed for completeness. Inspection of required forms was made and documented on the LIC 857 for children and the LIC 859 for staff. Criminal Record Clearances were reviewed for adults. CPR card expires in 04/09/2024 for Director. Staff present did have proof of the AB 1207 Mandated Reporter Training certificate on file.

CCC DID NOT COMPLETE TESTING PRIOR TO THEIR DEADLINE.
LPA referred Facility Representative to the Department website for lead: Lead Toxicity Prevention and Water Testing Information and PIN 21-21-CCP to obtain additional information and resources.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Licensee (or facility representative) was reminded that all adults 18 and over in the facility, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed facility. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation.

Based on this information, the following deficiency on the attached LIC 809D are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

A notice of site visit was given and must remain posted for 30 days.
