On 12/21/2022, Licensing Program Analysts (LPA) Lillian Casillas arrived at the facility to conduct an annual inspection at the infant center. LPA met with Director, Meredith Lapides, who guided LPA on a tour of the facility. LPA observed 5 infants with 3 teachers and 6 toddlers with 2 teachers during this inspection.

The facility is a single story building and shared with the preschool license #198013176. Care and supervision is provided in 2 classrooms, one dedicated to infants and another dedicated to toddlers. LPA observed age appropriate toys that are safe without sharp points, edges, splinters nor made of small parts that can be pulled off and swallowed. LPA observed the combination of toys provide manipulative skills and encourage auditory, visual and tactile stimulation. LPA observed 1 refrigerator each classroom. Each child's food and bottles are labeled. In the napping area of the infant classroom, LPA observed 7 cribs. Each crib is designated to one infant and is occupied by only one infant at a time. Cribs are kept free from all loose articles, including blankets and pillows. There are no objects hanging above or attached to the crib. Infants are not swaddled while in care. Infants up to 12 months of age are placed on their backs for sleeping. Facility demonstrated a sample of 15 minute infant sleeping logs (facility conducts 10 minute sleeping checks). Toddlers sleep on cots. LPA observed 2 diaper changing tables and 2 sinks in each classroom. LPA also observed a functioning carbon monoxide detector and a fire alarm system in each classroom. The 2A:10BC fire extinguisher is located in the hallway. LPA observed a fully enclosed infant play yard located directly in front of the infant classroom as well as fully enclosed outdoor play area dedicated to toddlers.. The outdoor play area includes astroturf, age-appropriate play equipment, and shade. Director stated infants also enjoy outdoor time on a 4 seater stroller.

LPA reviewed a sample of children’s files and observed files were complete, including the LIC 9227 Individual Infant Sleeping Plan and Infant Needs and Services Plan. LPA reviewed a sample of staff files and observed files were complete. Staff records contain documentation of meeting infant qualification requirements. [CONTINUE ON PAGE 2]

All staff are trained in CPR and Pediatric First Aid and possess current Mandated Reporter training certificates. There is a ratio of one teacher supervising no more than four infants and toddlers in care. The name of the child care center director or or fully-qualified teacher(s) designated to act in the director’s absence has been reported to the Department.

LPA verified that all adults present in the facility have obtained criminal record clearances and are associated to the facility. Prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption.



Incidental Medical Services (IMS) are currently being provided. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, no deficiencies are cited.

This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
