25 | At 09:42AM an unannounced Case Management-Incident inspection was conducted by Licensing Program Analyst (LPA) Lissete Gonzalez to follow up on an Unusual Incident. The incident was self reported via telephone on 08/23/2021 (Monday) and via fax on 08/26/2021 to Community Care Licensing. It was reported to the Department that the incident occurred at the facility on 8/20/2021 (Friday). At 10:32AM, Education Coordinator, Araceli Brion, arrived at the facility to conclude the inspection.
Upon LPA's arrival to the building at 09:42AM, LPA Gonzalez entered the facility and met with Lead Teacher, Inez Landin. The purpose of the inspection was announced to the Lead Teacher.
The Lead Teacher guided LPA on a tour of the facility. Census was taken. The following staff were present during this inspection: Room #01: 2 Staff with 13 preschoolers; Room #02: 3 Staff with 18 preschoolers; Room #03: 2 Staff with 11 preschoolers.
The Unusual Incident reported concerns of a possible violation of the Personal Rights of Child #1 by Staff #3 in Room #02. During today's inspection, LPA obtained documentation including the children’s roster and interviews were conducted with staff and witnesses. Due to insufficient information available at this time, the above allegation needs further investigation.
A printed copy of this report and appeal rights (LIC 9058) was provided to Education Coordinator, Araceli Brion, at the conclusion of the visit and their signature on this form acknowledges receipt of these forms.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. |
