25 | Licensing Program Analyst (LPA) Lissete Gonzalez conducted a Case Management inspection due to an incident that occurred on or about 08/20/21. Upon arrival, LPA met with AD of Educational Services & Center Operations, Gloria Mejia. Also present was Mentor Coach, Nneka Arinze who concluded the inspection with LPA L. Gonzalez. This is a subsequent inspection to the initial inspection conducted on 09/01/21. There were 49 children present.
The incident that occurred on 08/20/21 was reported to the Department via telephone on 08/23/21 and via fax on 08/26/2021. The facility reported the incident to the Department in a timely manner. The Unusual Incident, which was reported, was an allegation of a possible violation of the Personal Rights of a Preschool child.
LPA conducted interviews and obtained documentation including the facility roster and a copy of the police report from the Los Angeles County Sheriff’s Department. There were no disclosures to corroborate the allegation. This agency has investigated the allegation of a personal rights violation. Based on all of the information obtained and interviews conducted, no follow-up is necessary regarding the incident. There is not a preponderance of evidence to prove the alleged violation did or did not occur.
A notice of site visit was given and must remain posted for 30 days.
