Licensing Program Analyst (LPA) Lissete Gonzalez conducted an unannounced annual inspection on 08/12/2021 at 9:45AM. LPA met with Gabriela Rodriguez, Site Supervisor, who guided analyst on a tour of the facility. This is a Head Start preschool program which is operated by Volunteers of America of Los Angeles (VOALA). The program consists of two partial day sessions in Rooms 03 and 05. Session 1 operates from 8:00am to 11:30pm and Session 2 operates from 12:30pm to 4:00pm Monday through Friday. Note: There is a State program with license 198015579 on the same premises in Building B.

All areas identified on the Facility Sketch were inspected. Upon arrival, the following staff were present during this inspection: Room 03 (AM Session): Staff #1, #2 and #3 with 02 preschoolers; Room 05 (PM session) was not present during this visit. Facility was observed to be within the license capacity and limitations. The following was observed during the tour of the facility:

PHYSICAL PLANT
Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Children are not currently napping due to partial day schedules. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. General sanitation was observed. Availability of indoor drinking water was observed in classrooms.

Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Site Supervisor states there are no poisons at the facility. Carbon monoxide detectors were observed and are operable.

rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids or solid waste bags shall be discarded immediately after each meal.

This facility has a waiver for staggered outdoor play schedule. Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Availability of outdoor drinking water is provided via a water jug with disposable cups. LPA advised that no children shall be left without the supervision of a teacher at any time.

All floors were observed to me clean and safe. Al materials accessible to children were observed to be toxic free There are no firearms stored on the premises. There are no pools or bodies of water at the facility.

FACILITY RECORDS
All individuals present have obtained a criminal record clearance or criminal record exemption. There is at least one person trained in CPR and Pediatric First Aid present during this inspection.

Children’s Records were reviewed for completeness; Inspection of required forms was made and documented on the LIC 857.

Staff present had proof of the AB 1207 Mandated Reporter Training certificate on file. All staff have been given on the job training sanitation principles, housekeeping, including universal health precautions.

LPA did not review staff files during the inspection due to the staff files being stored at VOALA's Main Office located in North Hollywood located at 11243 Kittridge Street, North Hollywood, CA 91606. LPA will review staff files at a later time and date. If staff files were to be incomplete during the time and date that LPA reviews files, a case management inspection will occur at the facility to reflect the incomplete files.

Children's roster was reviewed and is current. Sign-In and Sign-Out sheets were reviewed. Children present were signed in. Disaster drill log was available, last drill was conducted on 4/16/2021.

Menus are posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. Snacks were reviewed for availability, quantity and appropriateness to children in care. The facility provides breakfast and lunch to children in the AM session; Lunch and a PM snack are provided to children in the PM session. Food is prepared by VOALA’s central kitchen (North Hollywood) and is delivered daily. All food items were observed to be individually packaged.

First Aid supplies were observed in the classroom in a cabinet. According to Site Supervisor, medication is only administered to a child when accompanied with a doctor's note and is stored in the office. The facility was observed to be equipped with an isolation area for any child who becomes ill and is located in the classrooms.

Medication: This facility provides Incidental Medical Services – IMS. Currently, there are no children enrolled that require storage of medication and equipment/supplies. LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov. UPDATE: LPA provided the Site Supervisor information on how to access Provider Information Notices (PINs) including Required Lead Testing: PIN 20-01 CCP Effects of Lead Exposure, and Guardian User Account Access - PIN 20-20-CCLD.

There were no deficiencies cited during today’s inspection.

Exit interview was conducted with Gabriela Rodriguez, Site Supervisor, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
