25 | ANNUAL INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced annual inspection to the above facility on 07/26/2022. LPA arrived at the facility at 10:35 AM and met with Meri Gevorgyan, Licensee who guided analyst on a tour of the facility. Also present during this inspection, was Sona Galstyan, Licensee’s Assistant. Per licensee, Meri Gevorgyan facility hours are Monday to Friday from 6:30 a.m. to 5:00 p.m. LPA provided a copy of Entrance Checklist—Family Child Care Home (LIC126) to licensee.
This is a one story home which consists of 3 bedrooms, 2 bathrooms, kitchen, dining room, living room (FIREPLACE: there is a fireplace which is inaccessible), converted garage for limited activity only, bathroom in the garage, storage in the backyard, front yard, and backyard (fenced). The children use the bathroom in the last bedroom adjacent to the daycare bedroom, dining room and living room (activity room) , 1 bedroom ( activity room & nap room), kitchen, fenced backyard, converted garage for limited activity only and bathroom in the garage. Per licensee, areas off limits to children and parents include: 2 bedrooms, 1 bathroom in the hallway of bedrooms, and front yard. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report.
All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. At 11:10 a.m., Licensee took this LPA on a tour of the home. LPA was toured through the interior starting with the living room and dining room (daycare classrooms), LPA observed children tables, chairs, and a piano in the living room, and observed a dining table, chairs, and sofas in the dining room. Next LPA toured the kitchen and observed child proof safety latches on the kitchen cabinets where licensee stores sharp items and cleaning compounds. Next LPA toured the one daycare bedroom and observed children toys, chairs, tables, and napping cots in the daycare bedroom. LPA observed all electrical outlets were covered in daycare areas.
