Licensing Program Analyst (LPA) Elka Chavez conducted an unannounced 1 Year Required inspection. LPA met with Director, Lucina Mena, who guided LPA on a tour of the facility. This is an infant program with a toddler option. Licensee serves toddlers ages 18 to 30 months old. The program operates Monday through Friday from with one session in the morning from 9:00AM to 12:00PM and an afternoon session from 12:30PM to 3:30PM. This facility licensed for 25 children. Per the Director there are 50 children enrolled.

At 10:45 AM all areas identified on the facility sketch were inspected. Upon arrival, LPA observed 19 children with 6 staff. Teacher-child ratios were observed to be in accordance with Title 22 regulations. The facility is within the conditions, limitations, and capacity specified on the license. Staff names were recorded. All children were observed to be under visual supervision of a teacher at all times.

Furniture and equipment were inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. All toilets and hand washing facilities are in safe and sanitary operating conditions. All materials and surfaces accessible to children are toxic free. At this time, the directors office is used as the designated ill isolation area. Parents are contacted immediately when children are determined to be ill. A first aid kit is kept in each classroom. Smoke detectors are present and fire extinguishers have been serviced in September 2022.

Snack menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. The facility provides AM and PM snack. The kitchen, food preparation, and storage areas are clean, free of litter, rubbish, and rodents/vermin. Children are provided with paper cups to drink bottled water. All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair. Disinfectants, cleaning solutions, poisons and other items that are dangerous to children are stored in an area inaccessible to children. Storage areas for poisons are locked.

Outdoor activity space equipment and surface is maintained in a safe condition as is free of hazards. Areas around and/or under climbing equipment and slides have cushioning material to absorb a fall. The Director states that there are no bodies of water on the premises and LPA did not observe any bodies of water during this visit. LPA observed the outdoor play area is fenced.

Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. Children’s records were reviewed for Admission Agreement, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents’ Rights, LIC 613-Personal Rights, LIC 702-Health History, and LIC 701 Physicians Report, and Immunizations Records. At 1:30 PM LPA observed child #1,3,4,5,6 & 12 missing immunization records.



Staff records were reviewed for Education, LIC-501: Personnel Record, LIC 508-Criminal Record Statement, LIC 9052- Employee Rights, LIC503- Health Screening, Proof of immunization against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse and Mandated Reporter Training Certificate. There is at least one person trained in CPR and Pediatric First Aid present during this inspection. LPA observed staff #1, 3,4,5 & 6 missing proof of MMR, Tdap, influenza and Mandated Reporter Training Certificate.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. Director, Lucina Mena was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

A notice of site visit was given and must remain posted for 30 days.

Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
