On March 13, 2025, at 9:25am, Licensing Program Analyst (LPA) Andrea Carter conducted an unannounced Annual / Random inspection and met with Facility representative (FR), Lucina Mena. LPA disclosed the purpose of the inspection and was granted entry into the facility.
At 9:35am LPA was taken on a guided tour of both the indoors and outdoors of the facility. Teacher-child ratios were observed, and staff names recorded. All center staff that were present during today’s inspection had fingerprint clearance and are associated to the designated license number. Care and supervision were evaluated to determine if the basic needs of children are met and appropriate. Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. Personal Rights of children were observed by LPA.
This is an infant program with a toddler option. Currently this facility does not have children under 12 months enrolled. The program operates Monday through Friday from 8:30am to 3:30pm. This facility offers two sessions 8:30am – 11:30am and 12:30pm – 3:30pm. Children attend 3 hours a day 3 days a week. This facility is licensed for a capacity of 25. At the initial start of the site visit, there is a total census of 16 children and a total census of 6 teachers present.

LPA observed all posting requirements for operation on the posting board and the following documents were observed: LIC 203A-License, LIC 610 A-Emergency Disaster Plan, LIC 9148-Earthquake Preparedness Check List, PUB 394-Notification of parent’s rights poster, LIC 613A- Personal Rights, PUB 269- Child Car Seat Law, Menus, and Activity Schedule.
Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting, and ventilation were evaluated. Napping equipment not observed, program is only 3 hours. Storage for children's belongings, and bathrooms facilities areas were inspected. Isolation area is located in the director’s office, supervised and away from other children in care. Age-appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, safety, and sanitation. Changing area was within the arm reach of the sink. LPA did not observe any hazards in the bathroom.

LPA observed cubbies with wooden toys, puzzles, books, and other appropriate toys to be clean and free of hazards at time of inspection. LPA observed carpets and age-appropriate tables and chairs in the classroom. LPA observed a red backpack for first aid kept in the classroom. First Aid supplies were inventoried and were found to be in compliance with regulation.
This facility provides a snack for each session. LPA observed snack menu at time of inspection. Snack items were observed to be properly stored. The kitchen, food preparation, and storage areas were clean, free of litter, rubbish, and rodents/vermin. Children are provided with paper cups to drink bottled water. All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair. Disinfectants, cleaning solutions, poisons and other items that are dangerous to children are stored in an area inaccessible to children. LPA confirmed Carbon monoxide and smoke detectors were present and determined to be in operable condition. Fire extinguishers were observed as serviced on 09/12/24. Disaster drill log observed, last drill conducted on 03/03/25 per observation.

At 9:45am, outdoor equipment was inspected for safety, cushioning material, good repair, and age-appropriateness. LPA observed bikes, sandbox area, sensory tables, a play structure with a plastic slide, and other age-appropriate toys around the play yard. Required shade and fencing were inspected. LPA advised center director that the children need to be within the direct care and visual supervision by the teachers at all times. The children have access to drinking water via water bottles and paper cups. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed at time of inspection.

At 10:15am, staff and children’s records were reviewed. Staff records were reviewed for Education, LIC-501: Personnel Record, LIC 508-Criminal Record Statement, LIC 9052- Employee Rights, LIC503- Health Screening, Proof of immunization against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse and Mandated Reporter Training Certificate. LPA observed current Pediatric CPR and First Aid Certification for Director CPR expires on 04/14/25 and Mandated Reporter was completed on 05/27/23 valid for 2 years.

· Staff #2 and Staff #5 did not have required immunizations on file.

Children’s records were reviewed for Admission Agreement, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents’ Rights, LIC 613-Personal Rights, LIC 702-Health History, and LIC 701 Physicians Report, and Immunizations Records. All required licensing forms were in files.

The following were discussed:
LPA advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation, this includes any changes as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.

To improve the quality and value of the new inspection process, a survey may be sent to the email address provided. Please complete the survey and share your inspection experience. Facility representative was reminded that all adults 18 and over in the facility, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed facility. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA referred facility representative to the Department website for lead: Lead Toxicity Prevention and Water Testing Information.

LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. MyChildCarePlan.org – Centers and Family Child Care Homes Licensee [or facility representative] was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Based on this information, the following deficiencies on the attached LIC 809D are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

A notice of site visit was given and must remain posted for 30 days.

Exit interview was conducted and report was reviewed with the Facility Representative, Lucina Mena.
