On 9/19/2024 at 12:35 pm Licensing Program Analyst (LPA), Claudia Kam made an unannounced required visit at above licensed facility. LPA disclosed the purpose of the inspection and was granted entry into the facility.

LPA met with director Laura Loza and was taken on a guided tour of both indoors and outdoors of the facility. LPA observed Teacher-child ratios, and staff names recorded. All center staff that was present during today’s inspection had fingerprint clearance and are associated to the designated license number. Care and supervision evaluated it is determined that the basic needs and personal rights of children are met and appropriate.

The hours of operation are 7:30 am – 5:00 pm, Monday - Friday. Facility is currently licensed for a capacity of 16. At the initial start of the site visit, there is a total census of 7 children and a total census of 6 teachers present. The facility is made up of 2 classrooms. Room 108 had 4 children and 3 staff and room 107 had 3 children and 3 staff present at time of inspection. LPA observed all posting requirements for operation on the posting board and the following documents were observed LIC 203A-License, LIC 610 A-Emergency Disaster Plan, LIC 9148-Earthquake Preparedness Check List, PUB 394-Notification of parent’s rights poster, LIC 613A- Personal Rights, PUB 269- Child Car Seat Law, Menus, and Activity Schedule.
Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting, and ventilation were evaluated. Children napping equipment met the requirement regulation at time of inspection. Storage for children's belongings, bathrooms facilities, and isolation areas were inspected. Age-appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, safety, and sanitation. Infant rooms have a Jack and Jill bathroom with 2 changing tables, 1 toddler size toilet and double sink with a separate sink for staff. Cubbies labeled with diapers for each child and belongings. LPA observed the classrooms to be equipped with cribs for each child, separate from the play area.

The play area has blocks, a quiet nook with cushions and books, cubbies with soft toys, and other appropriate toys were found to be clean and free of hazards at time of inspection. Disposable booties are worn by staff to keep the floors clean for infants. LPA observed carpets and age-appropriate tables and chairs in each classroom. LPA observed red packs for first aid kept in each classroom as well as a first aid kit kept in the bathroom. First Aid supplies were inventoried and were found to be in compliance with regulation. Sleep log, feeding log and diaper log were observed for children in care and found to be in compliance at time of inspection.

Facility provides breakfast, snack and lunch, menus were reviewed at time of inspection. All food items delivered daily to the facility by California Medical Hospital. Food and snack items are properly stored. No food is prepared on site. Bottles are delivered by the Hospital with pre-made formula for infants taking a bottle. Children who are eating baby food have food on site in sealed baby jars kept in the infant refrigerator. Drinking water is provided via tap water from the sink in the classroom. Carbon monoxide detectors and smoke detectors are present in the facility. LPA confirmed detectors at time of inspection and were determined to be in operable condition. Fire extinguishers were observed as serviced on 1/2024. Last fire drill was conducted on 8/9/24.



At 12:35 PM, LPA observed outdoor play area which consists of an infant structure, play house, push toys, tables and chair for activities, blankets for floor play. Outdoor equipment was inspected for safety, cushioning material, good repair, and age-appropriateness. Required shade and fencing were inspected. LPA advised Director that the children need to be always within the direct care and visual supervision by the teachers. Per Director, the children have access to drinking water via an outdoor sink and disposable paper cups. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed at time of inspection.

LPA reviewed with the Director the LIC 311A, Records to Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted. Staff and Children’s Records were reviewed. Criminal Record Clearances were reviewed for adults. LPA observed current CPR and First Aid Certification for center staff. CPR card expires on 3/30/2026 for Staff Member. Immunization records are not kept on site due to HIPPA, LPA has confirmed with director that all staff are vaccinated per regulation.

Mandatory Forms for the children’s files and staff files, requirements for fire drills, earthquake drills and documentation were discussed. Role and responsibilities of being a Mandated Reporter were reviewed. The Director was advised how to access forms and Regulations online at www.ccld.ca.gov. to ensure the most current information regarding regulations for all those who assists in providing care at the facility.

The following were discussed with the Director:
The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary. Smoke and Carbon Monoxide detectors should be checked, and batteries replaced as needed. Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location. The Director was also advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCLD within the time frame specified by the regulation.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Center was reviewed for lead compliance and center meets standards for lead. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

LPA discussed the safe sleep regulations with director Laura Loza and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

This facility plans to provide Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today 9/19/2024.

A notice of site visit was given and must remain posted for 30 days.
