25 | On 04/24/2024, Licensing Program Analyst (LPA) Stephanie Li conducted an annual continuation visit to Cortez Child development center at Pomona Unified Child development office. The purpose of this inspection is to review staff and children files as they were not available for review at the Cortez Child Development Center during the Annual inspection conducted on 04/18/2024. LPA met with Kym Allen, facility representative who provided LPA with the staff and children files who were present during the inspection.
All staff have received an active criminal record clearance as a condition of their employment with the Pomona Unified School District. Staff and children records were reviewed for completeness. Inspection of required forms were made. LPA issued the Review of Staff Records (LIC 859) and Children Records (LIC857) to the facility during this inspection. Lead teacher was missing mandated reporter certificate, citation being issued, see D page.
Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Based on Licensing staff interview and record review, the following deficiency listed on LIC809-D page are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health and safety.
A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative Kym Allen.
