Inspection was conducted in Spanish

On August 18, 2026 at 1:30 pm Licensing Program Analysts (LPA) Priscilla Ochoa conducted an unannounced annual inspection at the above facility. LPA was greeted by Licensee, Maria Juarez who guided LPA on a tour of the facility. Licensee’s assistant was also present during the inspection. Per licensee there are 10 children enrolled, and LPA observed 1 infant and 8 preschool aged children in care. Hours of operation are Monday - Saturday from 5:30 am – 5:00 pm.

Family members residing in the home are 2 adults who obtain criminal record clearance and four minor child. This is a single-story home that consists of 6 bedrooms, 2 bathrooms, living room, dining room, kitchen, detached garage, front yard and backyard (fenced). Licensee will submit an updated LIC279B.

Areas accessible to children in care are the living room, 2 bedrooms which are near the living room area, 1 bathroom in the hallway, and front porch.

At 1:45 pm licensee guided LPA on a tour of the facility. Per licensee, parents enter through the front door of the home which leads them to the living room. Upon entrance to the living room LPA observed age-appropriate toys for children in care. LPA also observed 6 napping children on mats. LPA then inspected the bedrooms that are accessible to children. Bedroom 1 was inspected and observed a playpen and multiple toys for children in care. Bedroom 2 was then inspected and observed 2 preschool aged children napping on a large bed. Per licensee, these children do not sleep on mats, which is why they sleep on a bed.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

LPA explained to licensee that children sleeping on a bed can pose a danger to their safety as they can roll over and fall. LPA explained that children need to be napping on cots or mats like the rest of the children. In bedroom 2, LPA also observed an infant napping in a play pen. LPA observed that there were toys, a blanket, 2 baby bottles and infant toy hanging over the play pen. LPA told licensee to remove all objects from inside of the crib and infant toy that was hanging over the playpen. LPA went over safe sleep pin with licensee as there was an immediate risk while the infant was napping. Per licensee, all bedding is washed at the facility weekly. The bathroom in the hallway was then inspected and was clean and orderly. LPA did not observe any hazardous materials that can pose a danger to children in care in the bathroom. Per licensee, the kitchen is inaccessible as it being remodeled. LPA observed a child safety gate and sheet blocking the entrance to the kitchen area. LPA explained to licensee that the department needed to be aware of the construction at the facility. Per licensee the construction should be finished by 08/18/2026. The front porch was then inspected and observed several age appropriate toys and outdoor equipment and children cubbies. All areas were observed to be clean, orderly, comfortable, and well ventilated.

There are first aid supplies available. Per licensee, the facility provides all meals for children in care. LPA reminded the licensee that food brought from home must be labeled with the child’s name, stored properly and or refrigerated.

If a child becomes ill, the child is isolated in bedroom 1 while the parents are called. A mat is available if needed.

Licensee's poisons, detergent, cleaning compounds, medications and other items which could pose a danger to child are stored where they are inaccessible to children.

LPA observed a smoke/carbon monoxide detector in the home, however LPA did not test detector as children were napping. LPA observed a 2A-10BC fire extinguisher in the living room area. The valve on the fire extinguisher was pointing to the fully charged green section and serviced on 03/25/2025. Per licensee they will have it serviced right away. There is a working telephone on site. Per Title 22 Emergency Disaster Drill must be conducted and documented at least every 6 months. LPA observed the last disaster drill to be conducted and documented on 7/10/26.

Per the licensee, there are no weapons, pets or bodies of water on the premises. Licensee obtains a current Pediatric First Aid and CPR certification which expires on 12/18/2027.

LPA conducted staff file review and deemed it not complete. 2 out of 2 staff did not obtain mandated reporter certificate for LPA to review. 2 out of 2 staff did not have proof of measles and pertussis immunizations for LPA to review and 1 out of 2 staff did not have proof of tuberculosis (TB) vaccine for LPA to review.

All childcare employees must complete mandated reporter training within 30 days of hire and renew training every two years at www.mandatedreporterca.com

Licensee has the required documents posted in the FCCH; Facility License (LIC 203), Notification of Parents' Rights Poster (PUB 394) and Earthquake preparedness Checklist.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, licensee confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

Ochoa informed licensee Maria Juarez that this report dated August 18, 2026 documents 1 Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA P. Ochoa informed the licensee Maria Juarez to provide a copy of this licensing report dated August 18, 2026 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. The facility is also being cited type B citations on the attached LIC 809D (deficiency page) in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health and safety.
