On 7/26/24 at 9:45 am Licensing Program Analyst (LPA), Claudia Kam made an unannounced required visit at above licensed facility. LPA disclosed the purpose of the inspection and was granted entry into the facility.

LPA met with Director Paquita Mansouri and was taken on a guided tour of both indoors and outdoors of the facility. Facility was closed to children during inspection and no children or staff other than director and receptionist were present. LPA has confirmed all staff that was present during today’s inspection had fingerprint clearance and are associated to the designated license number.

The hours of operation are 8:15am – 3:30pm, Monday - Friday with 2 half day programs 8:15am-11:15am and 12:30pm to 3:30pm. Facility is currently licensed for a capacity of 105. At the initial start of the site visit, there is a total census of 0 children and a total census of 0 teachers present. The facility is made up of 4 classrooms. Room 307, Room 308, Room 309 and Room 310. LPA observed all posting requirements for operation on the posting board and the following documents were observed LIC 203A-License, LIC 610 A-Emergency Disaster Plan, LIC 9148-Earthquake Preparedness Check List, PUB 394-Notification of parent’s rights poster, LIC 613A- Personal Rights, PUB 269- Child Car Seat Law, Menu and Activity Schedule was not posted as they are closed for the summer.

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting, and ventilation were evaluated. All equipment for children napping, play equipment, cubbies, and toys were disinfected and covered at time of inspection but were viewed as being present in the facility and to meet requirement regulation at time of inspection. Storage for children's belongings, bathrooms facilities, and isolation areas were inspected. Age-appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, safety, and sanitation. There are 2 toilets with a privacy partition and child size sink and adult sink with wooden step stool for hand washing for rooms 307 and 308.

Rooms 309 and 310 are currently not in use and are used for storage. LPA observed the rooms to be used for storage of supplies, holiday decor, and extra furnishings. LPA observed carpets and age-appropriate tables and chairs in each classroom. LPA observed a first aid cabinet with a lock kept in each classroom. First Aid supplies were inventoried and were found to be in compliance with regulation.

Breakfast, snack and lunch menus were not able to be reviewed at time of inspection as the facility is closed for the summer. Director states that the facility provides breakfast, lunch, and snacks. All food items delivered daily to the facility by a third party vendor and kept in the kitchen in a food warmer. LPA toured the kitchen and found in compliance for safety, cleanliness, and proper storage. Drinking water is provided via Brita water pitchers. Carbon monoxide detectors were not present at time of inspection. Smoke detectors were observed to be present in the facility. LPA confirmed detectors at time of inspection and were determined to be in operable condition. Fire extinguishers were observed as serviced on 6/26/2024.



At 11:00 AM, LPA observed outdoor play area which consists of a large shade structure with a garden, toddler size bridge, slide and play structure, a preschool play structure, tables for water play, tables for activities and tricycles. LPA observed a moving wall that is used to separate the play area for toddler and preschool use. At this time per director there are no toddlers enrolled. Outdoor equipment was inspected for safety, cushioning material, good repair, and age appropriateness. Required shade and fencing were inspected, the door to the parking lot was observed to be have an alarm sensor. LPA advised Director that the children need to be always within the direct care and visual supervision by the teachers. Per Director, the children have access to drinking water via Brita water pitchers. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed at time of inspection.

LPA reviewed with the director the LIC 311A, Records to Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted. Staff and Children’s Records were reviewed. Criminal Record Clearances were reviewed for adults. LPA observed current CPR and First Aid Certification for center staff. CPR card expires on 8/2025 for Staff Member. Mandatory Forms for the children’s files and staff files, requirements for fire drills, earthquake drills and documentation were discussed. Role and responsibilities of being a Mandated Reporter were reviewed. The Director was advised how to access forms and Regulations online at www.ccld.ca.gov. to ensure the most current information regarding regulations for all those who assists in providing care.

The following were discussed with the Director: The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary. Smoke and Carbon Monoxide detectors should be checked, and batteries replaced as needed. Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location. The Director was also advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.

Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Center was reviewed for lead compliance and center meets standards for lead. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.


Based on this information the following deficiencies on the LIC 809 D are being cited today 7/26/2024.

A notice of site visit was given and must remain posted for 30 days.
