25 | On 12/17/24 at 2:00 PM Licensing Program Analysts (LPA) Claudia Kam conducted an Unannounced Case Management Inspection due to a report in regard to physical plant update to the playground. LPA announced purpose of visit and initially met with receptionist who granted entry to facility. Also present at the facility was Lead Teacher Hamideh Kia who gave a tour of the facility.
The incident was reported to The Department on 11/15/24 via email by Facility Representative (FR), Karina Souquette updated Facility Sketch to Outdoor Playground in email and inquired about next steps.
During inspection, LPA obtained additional information regarding construction and action plan on relocation of children to limit impact on daily activities. Per FR, children have been using a enclosed garden area. Per FR, outdoor playground will be open to children upon approval of space by licensing. LPA conducted observation of outdoor play area and observed two age-appropriate play structures by Grounds for Play designed for ages 2-5 years. Both structures have shade, stairs, dual slides, and climbing apparatus. New cushioning material has been installed under each structure. LPA observed outdoor play areas for hazards and none were observed at time of inspection. Based on LPA's observations playground has been approved for usage. FR has reported incident and has made necessary action plan to ensure hazards are inaccessible to children and construction has had minimal impact to children's daily activities.
The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today, 12/17/24. A notice of site visit was given and must remain posted for 30 days.
Exit interview was conducted and report was reviewed with the Facility Representative Regional Manager, Sheila Wiley.
