Licensing Program Analysts (LPAs) Roxana Lopez and Lily Babcock conducted a case management inspection, following a complaint investigation on this date. LPAs met with Acting Director, Cindy Santos and Teacher Coordinator Myla Almorsolo.

LPAs conducted today's inspection for the purpose of a complaint investigation. During the complaint investigation the facility was inspected and the following was observed.

Upon arrival LPAs were informed that the previous director had resigned her position and that there is an acting director. During file reviewed it was determined that the Acting Director is not fully qualified to be a director and the proper documentation was not provided to the department.

During the inspection of the facility Licensing Staff observed four toilets in Classroom A. Two of the toilets were installed in the tub area. Per Acting Director the two extra toilets were installed sometime last year. The addition of the toilets was not reported to the department.

The following deficiencies listed on the attached 809D deficiencies page are being cited in accordance with California Code of Regulations Title 22

Exit interview was conducted with Acting Director Cindy Santos. A copy of this report and appeal rights was provided.



The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
