13 | Amended report as of 7/9/26. Allegation 1 & 2 amended. On June 30, 2026, at 1:35 p.m., Licensing Program Analyst (LPA) Keneisha Dunlap conducted an unannounced Complaint Inspection to deliver findings for the above pending allegation. LPA announced the purpose of the visit and was allowed entry into the facility by the Licensee- Angelica Jimenez.
During the course of the investigation LPA Dunlap conducted interviews with the Licensee, and parents.
It was noted that the facility staff violated the personal rights of C1.
Based on the investigation, including the Licensee and parent interviews, there is no documentation or corroborating witness statements to support whether a personal rights violation occurred.
Although an allegation may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violations occurred. Therefore, the allegations are deemed Unsubstantiated.
An exit interview was conducted with Licensee- Angelica Jimenez. The Notice of Site Visit given and must be posted for 30 days.
