Licensing Program Analysts (LPAs) P. Bishop and T. Tran arrived at the above facility for the purpose of an Unannounced Annual Visit at 12:30 pm. Upon arrival, LPA's announced the purpose of the visit and was granted entry into the facility by Director Ruzanna Davtian who provided tour of facility. LPA's provided the inspection Entrance Checklist, LIC 126. Per staff, the current hours of care provided are Monday-Friday 8:00 a.m. - 2:30 p.m. There were 37 children and 6 staff present during today's visit. All staff have current criminal background clearance.  LPA observed the facility license, Publication (PUB) 394- Notification of Parent Rights and Licensing Form (LIC) 9148- Earthquake Preparedness forms. Licensee was advised to post the required documents on the parent board. The disaster drill log was available during the inspection.  The last drill conducted was an earthquake drill done on 01/14/2025. 
At 1:15 pm LPA's were given a tour of the facility. The facility consists of three classrooms, restrooms for each classroom and and an outside play area. Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated.  Children have their own cubby to store their belongings.  Linens are taken home each week on Friday and returned on Monday. Per staff, the isolation area is located in in each classroom with Staff supervision. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms in each of the three classrooms. General sanitation was observed. Availability of indoor drinking water was observed in all classrooms and outside play area.
At 1;45 pm LPA's inspected the food service area. Staff stated that food was brought in to the facility and the meals that are served are Breakfast, Lunch and an afternoon snack. The refrigerator was checked for expired foods. LPA's observed that all food was not spoiled or expired. The food service was clean and proper sanitation procedures were being followed at the time of the visit.
Disinfectants, cleaning solutions and other items that are dangerous to children, were inaccessible to children.  Director states there are no poisons in the facility. Staff states that there are children present

Disinfectants, cleaning solutions and other items that are dangerous to children, were inaccessible to children.  Staff states there are no poisons in the facility. Staff states that there are children present receiving medication. Medications were inspected for proper storage that meet the regulation. According to the staff, medication will only be administered to a child when accompanied with a doctor's note and will be stored in the classroom of the child in the red backpack. Life saving devices  were inspected and found to be operable during inspection. Carbon monoxide detectors were observed inside of the classroom room. Per staff, there are no firearms stored on the premises.

Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. Perimeter fencing was observed around the outside play area. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards.  All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Availability of outdoor drinking water was observed. LPA's advised that no children shall be left without the supervision of a teacher at any time. There are no pools or bodies of water at the facility.

Children’s Records were reviewed for completeness. (The name, address, and telephone number of child’s authorized representative and medical assessment  are on file). Inspection of required forms was made and documented on the LIC 857. 

Staff records not available for review. Located at the main office will arrange to come back and review staff files.

Facility was informed of responsibility to report suspected Child Abuse by calling the Child Abuse Hotline at 1-800-540-4000. LPA's advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation, this includes any changes as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location.

LPAs informed the Facility Representative of the Child Care Advocate Program (CCAP) to provide many other helpful resources to the licensees and the public. LPA advised access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

Facility representative (FR), was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

CCC COMPLETED TESTING AND REMEDIATED LEAD EXCEEDANCES.
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

This facility provides Incidental Medical Services – IMS. LPAs reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records.

Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During today's visit, there were no deficiencies. A notice of site visit was given and must remain posted for 30 days. Appeal rights were given.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
