Licensing Program Analysts (LPAS) Franchesca White and Denise Gibbs arrived at the above mentioned facility for the purpose of an unannounced annual inspection. LPAS White and Gibbs were greeted by Licensee Johnnie Johnson and granted entry into the facility. Facility is currently licensed for a capacity of 25.The program currently operates Monday thru Friday from 6am to 6pm. Staff 1, 2, 3 have background clearance. 1 uncleared adult present at time of inspection. Uncleared adult working in the preschool classroom during the time of inspection. Gave the name of Geri Brown, and the Licensee corrected her and said her name was Geraldine Biree. Director states Geraldine Biree has not worked at the facility in a long time. Licensee Johnnie Johnson stated that the person's name was Lisa, then later said the person's name is Aisha Johnson.

At the initial start of the site visit, there is a total census of 11 Preschoolers and 3 School age children in the Preschool (3-4) class and a total census of 1 teacher and 1 Volunteer and one uncleared adult present. The facility is made up of four classrooms, the 2s Classrooms, Preschool, and School-Age. The facility is also attached to an Infant Center (198014887). At , LPAs toured 2s Classroom, furniture and equipment were inspected for age appropriateness and good repair, first aid kit observed in classroom hanging by door way. LPAS observed Preschool classroom to have a refrigerator near the fire extinguisher that has expired prune juice on the top shelf, and probiotics on the bottom shelf(pictures taken). LPAs observed roach and rodent traps under the reading table against the wall (picture taken). LPA's observed the Preschool bathroom to have two working toilets and three sinks in operable condition at time of inspection. LPAs observed toilet cleaners on the floor behind the toilets.

LPAS observed School age classroom to have cubbies filled with manipulatives (blocks, plastic toys), puzzles, books, dramatic play area, and desk and chairs for the children. School Age class has 1 restroom with one toilet and one sink with paper towels, and toilet paper.

The light switch in the restroom was inoperative at the time of inspection. LPAs observed roach and rodent trap under dramatic play area couch.

At 11:45 a.m., LPA White observed lunch time for the preschool children. LPA White observed Preschool teacher serve lunch to 9 children seated at the table. LPA observed Child 2 (C2) jumping off the couch with pizza in hand. LPA observed C1 tumbling off the couch and screaming while making steming motions with hands to mouth in a rapid motion. C1 repeated this same behavior the duration of lunch while the teacher called C1 by name. C1 never acknowledged Teacher.

At 12:25 p.m., LPA White observed C1 and C2 jumping from bed to bed, while preschool teacher called both C1 and C2 to stop and lay down. LPA White asked Teacher how she handles the behaviors of the children, she replied, I just let them jump and run. There nothing I can do."

Snack/lunch menus in the classroom were observed and are current. Food and snacks were reviewed for availability, quantity and appropriateness to children in care. Food preparation areas were toured for safety, cleanliness and proper equipment.

LPA observed required posted documentation outside of the classrooms, which included License, Publication (PUB) 393- Notification of Parent Rights, Licensing Form (LIC) 610- Facility Disaster Plan,and LIC 613A- Notification of Personal Rights and AM/PM snack and Lunch Menu. All documents were observed.
Facility records were reviewed for LIC 9040- Facility Roster, 9148- Earthquake Preparedness form, Daily schedule and Disaster drill log, last drill conducted on 6/23. All documents were observed. Carbon monoxide detector was tested and working properly at time of inspection. Fire Extinguisher was observed in the Preschool classroom to be fully charged with a service tag of 11/2022.

The Licensee and Staff have current Pediatric First Aid and CPR. Proof of immunization against influenza, and pertussis was readily available during today’s inspection. Proof of Mandated Reporter Training was not available for Staff 1 (S1), S3, S4.

Licensee Johnnie Johnson and Director Letichia Toney were reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES:
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for
drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.
For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).
LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Based on LPA's observations and records review, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children's health & safety.

A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent / guardian of any newly enrolled children for the next 12 months (1 year) The Acknowledgement form must be maintained in each child's file immediately upon receipt from parent. Licensee was provided with a copy of the parent Acknowledgement of Receipt of Licensing Reports Form during this visit.

The Notice of Site Visit (LIC9213) - must remain posted for 30 days. during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.

Meeting with the RO will be scheduled for a future date.
