Licensing Program Analyst (LPA) Franchesca White arrived at the above facility for the purpose of an unannounced annual inspection. LPA White announced the purpose of the visit and was granted entry by Johnnie McWilliams, while Director Leticha Toney was out retrieving today’s lunch for the daycare children. There are 12 children enrolled. There were five (5) day care children present at the time of inspection. There were three (3) staff present, and all have current criminal background clearance. The hours of operation are 6:00 a.m. – 6:00 p.m. (Monday – Friday). The facility shares a space with an infant toddler center (198014887). Upon entry of the school the following department documents were observed to be posted on the Parent Board: Facility License, Emergency Disaster Plan, Parents’ Rights, Emergency Evacuation Plan, Notice of Site Visits. Disaster Drill log was available at the time of inspection with the last drill being conducted on March 4, 2024.

At 10:00 a.m. LPA White observed five children playing on the yard. LPA observed two staff members to be with the children. A water jug and cups were available to children outside during play time.

The facility was inspected for the health, safety, cleanliness, heating, and ventilation for the children. LPA White observed the Preschool classroom to have age-appropriate tables and chairs for the children to sit for activities, and meals. LPA White observed the dramatic play area to have coat racks with purses, clothing items, and playhouse manipulatives, and a kitchen space for the children. There is a separate reading space between storage shelves filled with age-appropriate toys, and puzzles for the children with a circle time carpet centered between the storage shelves. LPA White observed an aquarium against the farthest wall near the exit to the playground. LPA White observed a refrigerator near the fire extinguisher in the preschool classroom to be stocked with Milk and cereal with visible expiration dates of April 21, 2024. In the seating area near the restroom LPA White observed a helium tank on the floor against the wall. This poses a potential risk to children in care. Directory Toney removed it from the classroom and placed it in an off-limits area making it inaccessible to children in care. LPA observed the mats and sheets to be reasonably clean with no excessive punctures exposing the foam material inside the mats.

At 10:30 a.m., LPA White checked the smoke/carbon monoxide dual detector located in the Preschool classroom, and it was found to be operable at the time of inspection. LPA observed two (2) 2A 10BC fire extinguishers both with current service tags (December 6, 2023)

At 10:45 a.m. LPA White observed the kitchen of the facility. LPA White observed the freezer to have frozen leftover food with no storage labeling on the Ziplock bags (photo taken). There was frozen bread with an expiration date of 3/31/2024 (photo taken). This poses a potential risk for children in care. LPA observed the items in the wall cabinets. LPA found syrup with an expiration date of August 22, 2023 (photo taken). This poses a potential risk to children in care. LPA White observed the menu to reflect that today’s lunch should be 1 baked chicken leg with mashed potatoes, corn, a slice of wheat bread, and milk. Johnnie McWilliams placed a post stick on the posted menu with a note stating that today’s lunch was Pizza, cucumber, grapes, and milk (photo taken).

At 11:00a.m., LPA White observed the preschool restroom to have three working sinks for washing hands, and two working toilets. There was no toilet paper available, but there were baby wipes in a container near the window seal of the restroom.

At 11:30 a.m., LPA White observed the outdoor play yard to have an age-appropriate play apparatus to have a slide and climbing bars. There are two wooden playhouses with stuffed animals and dolls inside. There is a large, shaded area near the slide. There are two lunch benches under the shaded area. The playground is completely fenced. There are tricycles and balls available for the children. LPA observed the play yard to be free of sharp objects, and flooring hazards.

At 12:00 p.m., LPA White reviewed the staff and children files. Staff 1,2,3 did not have current Mandated Reporter Training certificates available at the time of inspection. Staff 1,2,3 had current CPR/First Aid training certificates with an expiration date of June 2025. All staff had current immunization and TB records. LPA White reviewed children’s files and found them to have all department required documentation.

Licensee Leticha Toney was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for


drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.
For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).
LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Licensee LeTicha Toney was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Based on this information, the following deficiencies on the attached LIC 809D are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

A notice of site visit was given and must remain posted for 30 days.
