On Thursday, March 9, 2023 at 9:40 a.m., Licensing Program Analyst (LPA) Mayra Rivera conducted an unannounced annual inspection and met with director Claudia Garcia who guided LPA Rivera on a tour of the facility. During arrival, LPA Rivera observed 26 children present with 6 staff members.

This is a preschool program which consists of two classrooms (room 5, 6 and outdoor play yard). This preschool program operates from 8:30 a.m. to 12:00 p.m. half day. This site also houses a family literacy program and an infant program where mothers are taught parenting skills. The mothers stay on site with the infants. Both programs are exempt from Licensure. Sign in and out takes place at the entrance of the classrooms. All areas identified on the facility sketch were toured both indoors and outdoors. LPA Rivera observed 26 signatures and 26 children present during the visit.



At approximately 9:55 a.m. LPA entered classroom 5 to conduct an inspection. LPA Rivera observed 18 preschool children playing, and 3 staff members present. Teacher and child ratio were observed. LPA Rivera observed age appropriate furniture and equipment to be in good condition, free of sharp, not loose, or pointed parts and age appropriate toys accessible to children. LPA observed individual personal bin containers with children names inside their cubbies with their personal items. LPA observed cleaning solutions stored inside the storage cabinet with a child proof lock in place making it inaccessible for children to reach or open the cabinet door. LPA Rivera observed a step trash bin with a tight-fitting lid.

At approximately 10:06 a.m. LPA entered classroom 6 to conduct an inspection. LPA Rivera observed 8 preschool children doing circle time and 3 staff members present. Teacher child ratio were observed. LPA Rivera observed age appropriate furniture and equipment to be in good condition, free of sharp, not loose, or pointed parts and age appropriate toys accessible to children. with children names inside their cubbies with their personal items. LPA Rivera observed leaning solutions stored inside the storage cabinet with a child proof lock in place making it inaccessible for children to open. LPA observed a step trash bin with a tight-fitting lid.

Heating lighting and ventilation were evaluated by LPA Rivera. LPA observed central AC/heater and vents located on the ceiling wall. LPA observed the thermostat located in room 6 and the current room temperature to be at 70 degrees. LPA observed the fire extinguishers located inside the classrooms and the valve on the required 2A10BC fire extinguishers indicating fully charged and serviced 2/7/23. LPA Rivera observed Smoke/ carbon detectors in each classroom; tested and heard the sound. The combo smoke/carbon detectors are operable. LPA observed the fire and earthquake drill log. Last fire drill conducted on 11/28/22 and earthquake 12/14/22. LPA also observed the first aid kits in each classroom and fully stocked with band aids, gauzes, adhesive bandages and antiseptics. During this visit, per director Claudia the facility currently does not have children with medication or severe food allergies. LPA did not observe napping equipment because this program is only half day. For ill/isolation area children utilize the director's office and a cot is available for children to rest if needed. Lead testing has been conducted on 9/2/22 and facility has no lead level exceedance.

At approximately 10:23 a.m LPA Rivera observed the bathrooms (one located by room 6 and the other located by the administration office) to be in good condition with working toilets, sinks, hand soap dispenser, paper towels and toilet training seats. LPA observed a total of 6 toilets and 4 sinks. Ill/isolation area children utilize staff restroom located by the director's office.

At approximately 10:40 a.m. LPA Rivera entered the outdoor playground to conduct an inspection. LPA observed the equipment to be age appropriate and in safe condition, free of sharp, no loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under climbing equipment, are cushioned with granulated rubber mat padding that absorbs a fall. LPA observed the play structure to be for ages 2-5. For outdoor drinking water, the water jugs are available along with disposable water cups. For adequate shade, LPA observed a sun sail triangle. LPA observed 4 sheds and observed the sheds closed and locked with a key padlock. LPA did not observed bodies of water.

At approximately 10:54 a.m., LPA Rivera observed on the bulletin wall the license, Parents Right's, Personal Rights, LIC 610 Emergency Disaster Plan, Facility Sketch, LIC 9148 Earthquake Checklist, PUB 269 Child Passenger System Poster, monthly Menu and Daily Schedule.
At approximately 11:03 a.m. LPA Rivera reviewed staff files, children files, and children roster. Children and staff files are up to date and complete. There is at least one person trained in CPR and Pediatric First Aid Claudia Garcia present during this inspection. Educational background, training, and/or experience for each staff present are on file and were reviewed.

This program provides breakfast and lunch. The meals are provided by a vendor. A monthly menu is available and posted in each classroom. For water drinking LPA observed disposable water cups and water pitcher in each classroom. LPA observed the food preparation area to be clean and free of litter, rubbish, rodents, and/or any other vermin and storage containers for solid waste, including moveable bins with tight-fitting covers that are kept on, and in good repair. The facility was observed to be free of flies, other insects and rodents.

Director Claudia Garcia has proof of pediatric First Aid/CPR certification dated 8/19/2021, preventative health and safety training with lead component dated 4/21/22, child abused mandated certification (AB 1207) dated 3/26/21 and immunization against pertussis, and measles, and influenza declination. Director has been informed mandated reporter training must be completed every 2 years, and is available at www.mandatedreporterca.com


Items discussed during this visit:

1. The following items are an immediate citation and civil penalty of five hundred dollars ($500): Refused Entry to a Facility or Any Part of a Facility is a violation of Section 1596.852, 1596.853 or 1597.09. Regulations 101238 (g) (2), The Presence of an Excluded Individual, Fire Clearance Violations, Accessible Bodies of Water, Accessible Firearms, Ammunition or Both.
2. Breakfast/Lunch/Snack Menus: Menus are required to be posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days must also be available upon request.

3. Current Children’s Roster: Each child day care facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. This roster shall be available to the licensing agency upon request.
4. LIC 311A - Records to be Maintained at the Facility: Child Care Centers was provided to the applicant during this visit. LPAs advised that forms, regulations and quarterly updates can be accessed on the Child Care Licensing website at: www.ccld.ca.gov.
5.Pediatric First Aid and CPR: At least one person trained in Pediatric First Aid and CPR must be present.
6. Designated Staff: The name of the childcare center director or fully qualified teacher(s) designated to act in the director's absence must be on file.
7. Qualifications: Educational background, training, and/or experience for each staff present must be available for review.
8. Immunization Requirements: Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Director Claudia Garcia was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
