32 | During the tours, LPA observed cockroach traps in the kitchen cabinets and cabinets under the sink located in the classrooms. Additionally, LPA observed baking soda powder placed in the classroom cabinets under the sink and hallway cabinet where dry food supplies were stored. Per Director, it was placed for preventive measures.
LPA inspected the kitchen in presence of kitchen staff and the director. During the inspection of the kitchen cabinets, LPA observed a cockroach trap and two dead cockroaches inside the bottom drawer cabinet located next to the door. Photographs were taken. During today’s visit, LPA observed another cockroach in the cabinet under the sink in the classroom, which was cleaned by the director right away. Pictures were taken.
Per Director staff previously reported observing cockroaches in the kitchen area and nearby classroom. Information was confirmed by two staff members. Director stated that immediate steps were taken to address and prevent further pest activity. Director further stated that on April 24th, while facility was closed for the weekend , she and her husband conducted exterior spraying , cleaned interior areas and placed traps in the cabinets to prevent cockroach activity . Additionally, in classrooms 2A, 2B, and 3A all water dispensers and small carpets in front of the sink were replaced. Director stated staff are routinely reminded to maintain cleanliness, including cleaning classrooms and washing dishes. Director additionally stated that a cleaning crew cleans the classrooms daily, which was confirmed by staff members.
During this visit,Integrated Pest Management practices and professional pest assessment was discussed.
Based on LPA`s observations and interviews, which were conducted and a record review, the preponderance of evidence standard has been met, the above allegation are found to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division 12, Chapter 1) is cited on the attached LIC 9099D.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative.
An exit Interview was conducted, a copy of this report, along with Notice of Site visit, Deficiency page and Appeal Rights were explained and provided to the Facility Representative, Karine Vardanyan on 05/21/2026.
