On April 12, 2024, Licensing Program Analysts (LPA) Staicy Perry conducted an unannounced complaint investigation for the above allegation. A COVID-19 risk assessment was conducted prior to entering the facility. LPA met with Felicia Sanchez, Director, who guided LPA on a tour of the facility. LPA observed 22 children in care with 3 staff.

During the investigation, LPA interviewed Staff #1 (S1) to Staff #3 (S3). It was disclosed that snack menus have not been posted. LPA obtained a current facility roster, personnel roaster and photos.

Reporting Party (RP) alleged, "Snack menus are not being posted ". Per RP, The Reporting Party (RP) disclosed to LPA that the facility has not posted snack menus for a few weeks. Per RP, they have mentioned it to the center director, but menus have still not been posted. LPA interviewed S#1-S#3 who stated that snack menus are not posted and should be posted every month by supervisor or director.

During LPA Perrys inspection no snack menus were observed to be posted nor no menus were available at least one week in advance in an area accessible for review. Director was unable to provide copies of the menus or have kept on file for at least 30 days.

Based on interviews, observations and record reviews conducted, the above allegation is deemed SUBSTANTIATED. A finding that the complaint is substantiated means that the allegation is valid because the preponderance of the evidence standard has been met. California Code of Regulations, Title 22, Section 101227 (a)(6), Food Services are being cited on the attached LIC 9099D.

An exit interview was conducted, and a copy of this report was provided to Director Felicia Sanchez, along with Appeal Rights. A Notice of Site Visit was provided, Notice of Site Visit must be posted for 30 days

Per Director, prior to LPA leaving 4/12/24 a snack menu will be posted and a copy of menu will be sent to LPA via email. A written plan will be submitted to LPA on how they can ensure menus are being posted accordingly by POC date.

Based on observation, record review and interviews, LPA did not observe snack menus posted and it was disclosed by S#1-S#3 that snack menus are not posted accordingly, which poses a potential health, safety or personal rights risk to persons in care.
