25 | Licensing Program Analyst (LPA) conducted a case management inspection due to an incident that occurred on 12/28/22 . LPA met with Center Director, Felicia Sanchez, who guided LPA on a tour of the facility. At the time of inspection, LPA observed 15 children with 02 staff in the preschool room and 11 children with 03 staff from the Toddler 2 Room on the outdoor play yard.
LPA conducted interviews and obtained documentation during this visit.
The incident that occurred on Wednesday, 12/28/22 was reported to the Department on 01/04/23. On 12/28/22 C1 was taken to the doctor. however, the child's parents were notified that the child only had a chest contusion. The child was taken for further medical attention on 12/30/22 where the parents of C1 became aware that the child had a more serious injury. The facility was notified of this on 01/03/23 in the evening. The facility reported the incident the following day.
Based on all information obtained on this date, and interviews conducted with staff, no follow-up is necessary regarding the incident. The incident appears to be an unusual accident. It appears to be nothing the facility staff could have done to prevent the incident from occurring. LPA recommended that mats be placed over the metal plates that are located on the play yard to prevent any similar injury.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
There are no citations being issued today. Exit interview was conducted with Felicia Sanchez. Appeal rights were provided. |
