On April 12, 2024, Licensing Program Analysts (LPA) Staicy Perry conducted an unannounced complaint investigation for the above allegation. A COVID-19 risk assessment was conducted prior to entering the facility. LPA met with Elvira Valencia, Supervisor who guided LPA on a tour of the facility. Shortly after Felicia Sanchez, Director arrived and shortly after her arrival Executive Director, Michelle Johnson arrived. LPA observed 7 children in care with 3 staff.

During the investigation, LPA interviewed Staff #1 (S1) to Staff #5 (S5). who stated that snack menus have not been posted. LPA obtained a current facility roster, photos and personnel roaster.

Reporting Party (RP) alleged, " Snack menus are not being posted ". Per RP, The Reporting Party (RP) disclosed to LPA that the facility has not posted snack menus for a few weeks. Per RP, they have mentioned it to the center director, but menus have still not been posted. LPA interviewed S#1-S#5 who stated that snack menus are not posted and should be posted every month by supervisor or director.

Per S#4, they were informed to reuse snack menus. During LPA Perrys inspection no snack menus were observed to be posted, no menus were available at least one week in advance in an area accessible for review. Director was unable to provide copies of the menus nor kept on file for at least 30 days.

Based on interviews, observations and record reviews conducted, the above allegation is deemed SUBSTANTIATED. A finding that the complaint is substantiated means that the allegation is valid because the preponderance of the evidence standard has been met. California Code of Regulations, Title 22, Section 101227 (a)(6), Food Services are being cited on the attached LIC 9099D.

An exit interview was conducted, and a copy of this report was provided to Director Felicia Sanchez, along with Appeal Rights. A Notice of Site Visit was provided, Notice of Site Visit must be posted for 30 days.
