Licensing Program Analyst (LPA) Dayna Chambers conducted an unannounced random annual inspection to the above facility on 11/19/2024. LPA arrived at the facility at 1:00PM, identified self and met with Teacher Natalia Taylor at the door at 1:00pm. Norma Cueva, Master Teacher gave LPA a partial tour at 1:20pm, and Director, Natalie Ugalde arrived at the center approximately 1:25pm and completed the tour with LPA. LPA provided Licensee/facility representative with a copy of the LIC 125 Entrance Checklist to help facilitate the inspection. This is a preschool program which consists of 4 classrooms; Room #1 with 1 child and 1 staff, Room #2 with 8 children and 1 staff, Room #3 with 14 children and 2 staff, and Room #4 with 11 children and 1 staff. Room #1 had low enrollment and the child was combined with Room #4. This is a preschool program that has two shifts: AM shift starts at 8:00am to 11:15am and PM shift starts at 12:45pm to 4:00pm. The office hours are 7:00am to 5:00pm Monday through Friday. The facility was observed to be within the license capacity and limitations. The following was observed during the tour of the facility: Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting, and ventilation were evaluated. Children have their own cubby to store their belongings. Linens are taken home each week to be washed. Napping equipment (mats) were observed in a separate storage room. Per director, the isolation area is located in the office. Age-appropriate sinks and toilets were inspected for availability and good repair in all restrooms. General sanitation was observed. Availability of indoor drinking water was observed in classrooms. Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Director states that poisons are locked in each classroom in a cabinet on top shelf that is inaccessible to children. The center teachers clean and disinfect the center. Carbon monoxide and smoke detectors were observed and are operable. All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids or solid waste bags shall be discarded immediately after each meal. Outdoor playground equipment is in a safe condition,

free of sharp, loose, or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Availability of outdoor drinking water was observed. All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic-free There are no firearms stored on the premises. There are no pools or bodies of water at the facility. There is at least one person trained in CPR and Pediatric First Aid present during this inspection. Children’s Records were reviewed. The name, address, and telephone number of child’s authorized representative is on file. Inspection of required forms was made and documented on the LIC 857. LPA also reviewed staff records. The review of Staff records was documented on the LIC 859. Staff present did have proof of the AB 1207 Mandated Reporter Training certificate on file. The center utilizes a master file for YMCA which is not present at the center address. The main office is located at 4801 Airport Plaza Drive, Long Beach, CA 90815. (562-279-1622). LPA will check at a later time for staff immunizations and licensing forms. All staff have been given on the-job training sanitation principles, housekeeping, including universal health precautions. This center utilizes to apps called Classroom Dojo and Care Connect. Director printed out the roster report and sign in/Sign Out for review. Children's roster was reviewed and is current. Sign-In and Sign-Out sheets were reviewed. Children presents were signed in. LPA provided Director a copy of the Title 22 regulations regarding signing in/out to ensure the fields are covered in the app when printing out the report. Each classroom has their own disaster drill log. The classrooms all do their drills on various dates. Room #1 last drill was completed on 11/12/2024. Menus are posted one two weeks in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. Snacks were reviewed for availability, quantity, and appropriateness to children in care. This center provides breakfast for the AM shift and a PM snack for the PM shift. First Aid supplies were observed on the wall in each classroom. The center provides medications to children. LPA requested the Director update the IMS statement. According to the Director, medication is only administered to a child when accompanied with a doctor's note and is stored in the office. transfer their existing clearance or exemption, prior to initial presence in a Childcare Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Childcare Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP).LPA verified that the lead testing was completed in accordance with the Written Directives outlined in PIN 21-21.1-CCP. The center conducted their lead testing and report is dated 08/19/2022. There were no lead exceedances.LPA referred the Director to the Department website for lead: Lead Toxicity Prevention and Water Testing Information. This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and

Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

A notice of site visit was given and must remain posted for 30 days.
