On July 12, 2022 at 1:28 PM, Licensing Program Analysts (LPAs) Monique Ayala and Veronica Martinez-Garza conducted an unannounced Required 1 Year inspection at the above facility. A risk assessment was conducted prior to entering the facility. Upon arrival LPAs were greeted by licensee, Amparao Nunez. LPAs observed 14 children in care. LPA observed Child Care Facility Roster. LPA observed 14 children files contained all required licensing documents. The licensee is operating within proper capacity and ratios. LPAs observed licensee to be present at the home and providing adequate care and supervision. Hours of operation are Monday through Friday, 8:00 AM to 5:00 PM.

The home is single story house with 2 bedrooms and 1 bathroom, living room, dinning room, kitchen, laundry room, front yard and rear yard. LPAs observed cleaning compounds inaccessible to children in care.

The day care takes place in living room, dinning room, bedroom 1, bathroom located on the right side of the hallway and hallway bathroom and front yard. The home is clean, orderly, comfortable and well ventilated.

Licensee's poisons, detergent, cleaning compounds, medications and other items which could pose a danger to child are stored where they are inaccessible to children.

LPAs observed working smoke detector and Carbon Monoxide, fully charged fire extinguisher and working telephone. There are several age appropriate toys and a first aid kit on the premises. Per the licensee, there are no firearms on the premises. The licensee has current CPR and first aid that expires 05/21/2024.

During this inspection LPAs observed C1 sleeping in a playpen with blankets placed over the playpen, blankets in the playpen and an attachment on the pacifier. Licensee removed all blankets and pacifier attachment. LPAs did not observe sleeping logs for any of the infants that were in care. LPAs observed 2 exausers in the front yard that are used for the infants per licensee. Licensee removed the exausers from the day care and moved them to rear yard that is inaccessible to children in care.

The licensee has an expired mandated reporter training at www.mandatedreporterca.com. Certificate expired on 01/30/2021. All childcare employees must complete mandated reporter training within 30 days of hire and renew training every two years



The licensee has the required immunizations.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department.

Licensee was advised of the requirement to report unusual incidents and/or injuries to the parent/guardian and Licensing within the time frame specified by the regulation and on the form LIC624B

The licensee was informed of the responsibility to report suspected Child Abuse by calling the Child Abuse Hot-line at 1-800-540-4000. Also call the CCL office and follow up with a written Unusual Incident/Injury Report (LIC 624B).

The licensee was informed that the presence of adults in the home without Criminal Record Clearance or Exemption will be cited and civil penalty assessed for $100 per day. requirements for disaster drills (fire and earthquake). Mandatory Forms for the children’s files and provider’s files, and Safe Sleep Awareness. The role and responsibilities of being a mandated reporter were reviewed. Licensee was reminded that supervision is always required to children in care.

Licensee was made aware that it is their responsibility to know the regulations as well as anyone who assists in providing care. Licensee was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care. Licensing must always have the facility’s phone number; if the phone number is changed, licensing must be notified.

Regulation prohibits the smoking of tobacco in a private residence that is licensed as a family childcare home and in those areas of the family day care home where children are present (24/7 ban). State law prohibits baby walkers, bouncy seats, exersaucers and any other items that fall into that category.

A copy of Safe Sleep Regulations was provided to Licensee.

LPA provided consultation during inspection.

The following Type A's and Type B's deficiencies were cited in accordance to Title 22 of the California Code of Regulations and/or Health & Safety codes. Refer to LIC809D

Upon receipt of this report documenting a substantiated complaint allegation and a Type A deficiency, the licensee shall do the following:


1. Post the Notice of Site visit and any licensing report documenting a Type “A” deficiency.
2. The report and the Notice of Site visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty.
3. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year).
4. The Acknowledgement form (LIC 9224) must be maintained in each child’s file immediately upon receipt from parent. A copy of the parent Acknowledgement of Receipt of Licensing Reports Form was provided during this visit.

An exit Interview was conducted, a copy of this Report and a Notice of Site visit was provided to the licensee along with appeal rights.
