Licensing Program Analysts (LPAs) Rosaura Valenzuela and Alicia Mooberry conducted an unannounced subsequent visit for the above noted allegations. LPAs met with Adriana Velazco. The purpose of the visit was to deliver the findings of the complaint investigation.

It was alleged that staff are not allowing day care children’s parents into the facility. To investigate this allegation on 5/9/2023 between 1:10pm and 1:45pm, staff interviews were initiated. Staff interviews revealed that parents were not and are still allowed to enter the facility. They are not allowed to be inside the facility due to a management decision regarding Covid-19. The facility will begin to allow parents inside starting on July 1, 2023.
Based on interviews, there is sufficient information to support the allegation. Therefore, this allegation is being SUBSTANTIATED at this time.

Deficiencies cited based on Title 22 regulations.

Licensing Program Analysts (LPAs) Rosaura Valenzuela and Alicia Mooberry conducted an unannounced subsequent visit for the above noted allegations. LPAs met with Adriana Velazco. The purpose of the visit was to deliver the findings of the complaint investigation.

It was reported that facility is understaffed. To investigate this allegation on 6/7/2023 at 11:30am, LPAs visited two classrooms. The facility is currently licensed for 43 children. There is also an infant license with a toddler component. There were 4 staff present and 28 children in care. Between 1:10pm and 1:45pm, staff interviews were initiated. Staff interviews revealed that at times they are short-staffed, but that they find ways to stay within ratio. For example, the site director will contact Child Careers to obtain substitute teachers and aides. If they need more coverage, then the site director will step in and provide coverage. Children can also be moved to another class that does not have a lot of students. On 6/7/2023 the facility was observed to be within teacher-child ratios. On 6/16/2023, the facility was observed to be within ratio.

Based on observation and interviews, there is not sufficient information to support this allegation. Therefore, this allegation is deemed UNSUBSTANTIATED at this time.

It was alleged that the facility is in disrepair. To investigate this allegation on 6/7/2023 at 11:30am, a physical plant tour was conducted. LPAs observed the facility to be clean and in good condition. The facility was built in 2009.

Based on observation there is not sufficient information to support this allegation. Therefore, this allegation is deemed UNSUBSTANTIATED at this time.

It was reported that staff did not notify day care children’s parents of construction. To investigate this allegation on 6/7/2020, between 1:10pm and 1:45pm, staff interviews were initiated. Interviews revealed that parents were notified verbally but not in writing that construction was going to be conducted. On 5/9/2023, the facility notified the Department that a plumbing company was hired to replace pipes. During the time of construction, children were not allowed to place in the outside yard. Students had psycho-motor activities in the classrooms.

Based on interviews and records review there is not sufficient information to support this allegation. Therefore, this allegation is UNSUBSTANTIATED at this time.

Exit interview conducted and a copy of the report was issued.
