On January 29, 2025, at 10:05am, Licensing Program Analyst (LPA) Keneisha Dunlap arrived at the above facility for the purpose of an unannounced Annual Inspection. LPA Dunlap announced the purpose of the visit and was granted entry into the facility by Site Director - Marben Perancullo. There are 105 children enrolled, 89 children present, and 15 staff at the time of inspection. The hours of operation are Monday- Friday from 7:00am- 5:00pm. This is a preschool program serving children 3-5 years of age. All adults at the facility were discussed and background and fingerprinted cleared.

LPA observed all posting requirements for operation on the licensing board LIC 203A-License, LIC 610A, PUB 393 LIC 9040, PUB 269, LIC 9148, LIC 500, LIC 613A, Menus, Activity Schedule, and completion testing and remediated lead exceedances.


Documentation of current Fire & Earthquake drill was observed with last drill on 10/14/24.
Fire extinguisher observed with a tag date of 12/17/24.

LPA observed a operable fire system, and carbon monoxide detector.

The center has a HVAC system throughout the facility.

LPA Dunlap observed an inventoried first aid kits in the classroom, and playground area.



LPA Dunlap observed sign in and out sheets for all classrooms with all required signatures.

All 2, 3, 4 and 5 classroom have age appropriate toys and furniture as well as a carpeted areas, library, arts & crafts, manipulative play, and dramatic play areas. Each classroom has their own water dispenser with disposable cups readily available for children in care to use. Each classroom has trash cans with tight fitting lids. All classrooms have cots stored in classroom. All electrical outlets have covers. LPA observed the Page 1 of 5

furniture and equipment in good condition, free of sharp, not loose, or pointed parts and carpet area clean. All cleaning supplies are stored in cabinets with safety latches and/or locks with keys.

The 2 bathrooms in hallways are age appropriate sinks and toilets. All bathrooms were found clean and in good working good condition.

Classroom 1 provides care for 3 year olds. It has age appropriate toys and furniture as well as a carpeted areas, library, arts & crafts, manipulative play, and dramatic play areas. Classroom has their own water dispenser with disposable cups readily available for children in care to use. Classroom has trash cans with tight fitting lids. All electrical outlets have covers. Cots are stored in classroom. LPA observed the furniture and equipment in good condition, free of sharp, not loose, or pointed parts and carpet area clean. All trash cans have tight fitting lids. All cleaning supplies are stored in cabinets with safety latches and/or locks with keys.

At 10:35am, LPA Dunlap observed in Classroom 1 the portable sink w/ ABA Basin faucet not working. LPA Dunlap informed the Site Director, and the water tank was refilled. This poses a potential health, safety or personal rights risk to persons in care. A Technical Violation will be issued.

The outside playground has a outdoor shaded area, grass, bikes, blocks, art supplies, picnic tables, climbing structures, and age appropriate toys. The playground has 2 hand washing sinks that have drinking faucet readily available for children in care to use. LPA observed the outdoor area and outdoor equipment in good condition, free of sharp, not loose, or pointed parts.

At 10:45am LPA Dunlap observed a pruner on sand table on playground. Site Director removed during visit. This poses a potential health, safety or personal rights risk to persons in care. A Technical Violation will be issued.

The kitchen area is is located in adjacent building. The kitchen area is clean and all equipment in good and working condition. There were no expired or rotten food found at the time of the inspection.



At 11:00am, LPA Dunlap observed shelf food items has not been labeled with mask tape on outside of labels and jars with dates. All food in refrigerator and freezer have not been labeled with dates written with a marker, and/or mask tape with date on the outside. This poses a potential health, safety or personal rights risk to persons in care. A Technical Violation will be issued. Page 2 of 5

The children do nap.

The isolation area is located in the Directors office.

The Director stated they do not have children that they administer medication to at this time. If children were to have medications that need to be administered there is a lockbox located in classroom.



The Director stated there are no firearms at the facility.

The Director stated that there are no smokers at the facility.

The Director stated that they do not provide transportation for children.

There are no large bodies of water on premises.

Criminal Record Clearance - Child Care Centers Site Director - Marben Perancullo was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
