On 07/11/2024 at 3:30 PM Licensing Program Analysts (LPAs) Katrina Chicote and Peter Bishop conducted an Unannounced Case Management inspection to follow up on an incident that was reported to the Department on various dates, this inspection is a follow up visit as initial follow up was already conducted on 05/08/2024. Upon arrival, LPAs met with Facility Representative (FR), Mea Smith, Head Teacher, who provided LPAs on a tour of the facility inside and outside. Census was taken.

During this inspection LPAs interviewed FR who worked in the classroom where child was assigned. FR states that she did work directly with the child as part of PCC program. FR states that PCC program and Teacher for that class is currently off for the Summer. FR states she is familiar with incident and the child and did not observe any incidents involving child or any staff member at the facility. FR confirms child no longer attends and is no longer enrolled. FR states that she did observe child's Authorized Representative make verbal threats towards the teacher, which resulted in that teacher calling LAPD. FR states child's Authorized Representative made allegations in regards to her child's personal rights being violated but never disclosed who the staff member was. Departments interviews corroborate this statement. MPSW RO partnered with IB to follow up with incident and interview child and child's Authorized Representative but they were not able to obtain any additional information and finding was No Further Action Required.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today, 07/11/2024.

A notice of site visit was given and must remain posted for 30 days.

Exit interview was conducted and report was reviewed with the Facility Representative, Mea Smith.
