25 | On 02/19/2025 at 2:35 PM Licensing Program Analysts (LPA) Claudia Kam and Joshua Ortega conducted an Unannounced Case Management - Incident inspection to follow up on an incident that was reported to The Department on 12/13/24. Upon arrival, LPAs met with Office Staff, who granted entry to facility. LPAs announced purpose of inspection and met with Facility Representative (FR) Ana Vidal, Principal. Census was taken.
On 12/13/24, an incident was reported to The Department that a child was injured at the facility. The facility reported this incident to The Department within the required 24 hours and submitted a written report via email.
During this inspection LPAs interviewed FR, who reported the incident to Licensing. LPAs observed the location of the accident and observed child in classroom unhindered. LPAs observed playground to be maintained with no visible hazards. Child doctor note was observed. Doctor note reflects that the child was taken for medical attention and released to return to school the next school day. Child was not diagnosed for an injury requiring treatment.
Based on interviews conducted and record reviewed, it appears that the facility had followed the health and safety protocol to ensure the safety of children in care. Based on the available information it does not appear this incident was the result of a Title 22 violation for lack of care and supervision.
The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today, 02/19/2025.
A notice of site visit was given and must remain posted for 30 days.
Exit interview was conducted and report was reviewed with the Facility Representative, Ana Vidal.
